艾灸联合大黄芒硝穴位贴敷治疗乳腺癌相关性淋巴水肿的临床研究

摘　　要：目的探究艾灸联合大黄芒硝穴位贴敷治疗乳腺癌相关性淋巴水肿的临床疗效及对患者血清炎症因子水平及肩关节活动度的影响。方法选取2019年9月至2022年9月赣州市肿瘤医院收治的116例乳腺癌相关性淋巴水肿患者作为研究对象,按照随机抽签法分为对照组与观察组,每组58例。对照组采用常规西医疗法治疗,观察组在对照组基础上给予艾灸联合大黄芒硝穴位贴敷治疗。比较两组临床疗效、中医症候积分、炎症因子、肩关节活动度及不良反应发生率。结果观察组治疗总有效率为91.38%,高于对照组的75.86%,差异有统计学意义(P<0.05);治疗后,两组水肿疼痛、肢体麻木、乏力纳呆、心烦易怒评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组细胞介素-17、Toll样受体-4、淀粉样蛋白A水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组外展、内收、前屈、后伸活动度均大于治疗前,且观察组大于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较差异无统计学意义。结论艾灸联合大黄芒硝穴位贴敷治疗乳腺癌相关性淋巴水肿效果显著,可有效降低炎症因子水平,提高患者肩关节活动度,安全性良好,值得临床推广应用。Objective To investigate the clinical efficacy of moxibustion combine with rhubarb mirabilite acupoint application on breast cancerrelated lymphedema and its effect on serum inflammatory factor levels and shoulder joint mobility.Methods A total of 116 patients with breast cancer-related lymphedema admitted to Ganzhou Tumor Hospital from September 2019 to September 2022 were selected as the research subjects,they were divided into the control group and the observation group according to random drawing method,with 58 cases in each group.The control group was treated with conventional Western medicine therapy,and the observation group was treated with moxibustion combined with rhubarb and mirabilite acupoint application on the basis of the control group.The clinical efficacy,Traditional Chinese Medicine symptom score,inflammatory factors,shoulder joint activity and incidence of adverse reactions were compared between the two groups.Results The total effective rate of treatment in the observation group was 91.38%,which was higher than 75.86%in the control group,and the difference was statistically significant(P<0.05);after treatment,the scores of edema pain,limb numbness,fatigue,anorexia and irritability of the two groups were lower than those before treatment,and the observation group was lower than the control group,and the differences were statistically significant(P<0.05);after treatment,the levels of interleukin-17,Toll-like receptor-4 and amyloid A of the two groups were lower than those before treatment,and the observation group was lower than the control group,and the differences were statistically significant(P<0.05);after treatment,the range of abduction,adduction,flexion and extension of the two groups were greater than those before treatment,and the observation group was greater than the control group,and the differences were statistically significant(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups.Conclusion Moxibustion combine with rhubarb mirabil
