超声引导下经皮球囊扩张术在自体动静脉内瘘狭窄治疗中的应用效果

摘　　要：目的探讨超声引导下经皮球囊扩张术(percutaneous transluminal angioplasty,PTA)在自体动静脉内瘘(autologous arteriovenous fistula,AVF)狭窄治疗中的应用效果。方法选取2021年1月至2023年5月上饶市人民医院收治的80例伴AVF狭窄的维持性血液透析(maintenance hemodialysis,MHD)患者作为研究对象,按照手术方式不同分为研究组与对照组,每组40例。对照组接受传统外科手术治疗,研究组接受超声引导下PTA治疗,比较两组手术成功率、并发症发生率、内瘘相关指标、AVF再狭窄发生率。结果两组手术成功率均为100.00%。术后,研究组狭窄处内径长于对照组,内瘘自然血流量、透析血流量均多于对照组,差异有统计学意义(P<0.05)。研究组术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。术后6个月,研究组AVF再狭窄发生率低于对照组,差异有统计学意义(P<0.05)。结论超声引导下PTA治疗MHD患者AVF狭窄,可更好地修复内瘘功能,降低术后并发症和AVF再狭窄发生率,值得临床推广应用。Objective To investigate the application effect of ultrasound-guided percutaneous transluminal angioplasty(PTA)in the treatment of autologous arteriovenous fistula(AVF)stenosis.Methods 80 maintenance hemodialysis(MHD)patients with AVF stenosis admitted to Shangrao People's Hospital were selected as the study subjects,they were divided into the study group and the control group according to different surgical methods,with 40 cases in each group.The control group received traditional surgical treatment,and the study group received ultrasound-guided PTA treatment,the success rate of surgery,incidence of complications,internal fistula related indicators,and incidence of AVF restenosis were compared between the two groups.Results The success rate of operation were 100.00%of the two groups.After operation,the stenosis internal diameter in the study group was larger than that in the control group,and the natural blood flow of internal fistula and dialysis blood flow were more than those in the control group,and the differences were statistically significant(P<0.05).The incidence of postoperative complications in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).At 6 months after surgery,the incidence of AVF restenosis in study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Ultrasoundguided PTA in the treatment of MHD patients with AVF stenosis can better repair internal fistula function,reduce the incidence of postoperative complications and AVF restenosis,which is worthy of clinical promotion and application.
