摘 要:目的:分析电子计算机断层扫描(CT)影像组学鉴别诊断肺部磨玻璃结节良恶性的价值。方法:选择2020年1月~2023年1月收治的肺部磨玻璃结节患者80例病历资料进行回顾分析。训练组(n=56)均接受病理组织检验,以其结果为准分为良性组(n=28)、恶性组(n=28),验证组(n=24)。比较训练两组患者临床特征,并评估训练组患者肺部磨玻璃结节的常规CT影像特征,采用Lasso回归筛选影像特征与预测肺部磨玻璃结节良恶性最相关影像组学特征,比较恶性组和良性组临床特征的差异性。同时,以Logistic回归建立预测模型,分析肺部磨玻璃结节良恶性的预测因素。结果:恶性组分叶征、毛刺征、支气管征及囊状透亮影率高于良性组,病灶密度低于良性组(P<0.05);Logistic回归分析证实:临床特征的CT值、临床特征得分和影像组学得分等可作为肺部磨玻璃结节良恶性预测因素(P<0.05)。结论:CT影像组学有效鉴别诊断肺部磨玻璃结节的良恶性,有助于医师准确拟定个性化治疗方案,对患者病情改善有重要意义。Objective:To analyze the value of electron computed tomography(CT)imaging histology in the differential diagnosis of benign and malignant ground glass nodules in the lung.Methods:The medical records of 80 patients with pulmonary grinding glass nodule treated from January 2020 to January 2023 were retrospectively analyzed.All the training group(n=56)underwent pathological examination,and were divided into benign group(n=28),malignant group(n=28),and verification group(n=24)according to the results.The clinical features of the two groups were compared,and the conventional CT image features of the pulmonary glass grinding nodules in the training group were evaluated.Lasso regression was used to screen the image features and predict the most relevant image omics features of the benign and malignant pulmonary glass grinding nodules,and the differences in clinical features between the malignant and benign groups were compared.At the same time,the prediction model was established by Logistic regression to analyze the predictive factors of benign and malignant pulmonary grinding glass nodules.Results:The malignant group had a higher rate of lobar sign,burr sign,bronchial sign and cystic translucent shadow than the benign group,and the density of lesions was lower than that of the benign group(P<0.05);Logistic regression analysis confirmed that the CT values of clinical features,clinical feature scores,and imaging histology scores could be used as predictive factors for the benignness of ground-glass nodules in the lungs(P<0.05).Conclusion:CT imaging histology effectively identifies and diagnoses the malignancy and benignity of pulmonary ground-glass nodules,which helps physicians accurately formulate personalized treatment plans and is of great significance to the improvement of patients’conditions.
