摘  要:目的:探讨多层螺旋CT与磁共振成像(MRI)在膝半月板和关节软骨损伤的应用效果及准确率。方法:选取骨科收治的184例膝关节半月板和关节软骨损伤患者,应用分层随机抽样法将患者分为MR组与CT组,每组92例。CT组应用64排螺旋CT诊断,MR组应用3.0TMRI系统对患者进行诊断。比较两组检查完成时间与临床确诊时间、两组误诊及漏诊率、两组分型诊断准确率以及两组诊断效能。结果:CT组完成时间(6.52±2.21)优于MR组(12.38±5.24)(P<0.05);但MR组临床确诊时间优于CT组(P<0.05);MR组误诊率及漏诊率均低于CT组(P<0.05);CT组和MR组在对膝关节半月板和关节软骨损伤的Ⅰ期、Ⅱ期和Ⅲ期进行诊断时,准确率差异不显著(P>0.05);MR组诊断膝关节半月板和关节软骨损伤的灵敏度及排除膝半月板和关节软骨损伤能力优于CT组(P<0.05)。结论:CT在膝关节半月板和关节软骨损伤的诊断中,除了完成时间优于MRI以外,在确诊时间、误诊及漏诊率以及诊断效能等均不及MRI组,MRI在膝半月板和关节软骨损伤的诊断中具有重要的诊断价值。Objective:To explore the application effect and accuracy of multi-slice spiral CT and magnetic resonance imaging(MRI)in knee meniscus and articular cartilage injury.Methods:184 patients with knee meniscus and articular cartilage injury were selected from orthopedics department,and they were divided into MR group and CT group by stratified random sampling,with 92 cases in each group.CT group was diagnosed with 64-slice spiral CT,and MR group was diagnosed with 3.0T MRI system.The time of examination completion and clinical diagnosis,the rate of misdiagnosis and missed diagnosis,the accuracy of two-component diagnosis and the diagnostic efficiency of the two groups were compared.Results:The completion time of CT(6.52±2.21)was better than that of MR group(12.38±5.24)(P<0.05).However,the time of clinical diagnosis in MR group was better than that in CT group(P<0.05).The misdiagnosis rate and missed diagnosis rate in MR group were lower than those in CT group(P<0.05).There was no significant difference in the accuracy between CT group and MR group in the diagnosis of meniscus and articular cartilage injury of knee joint in stageⅠ,ⅡandⅢ(P>0.05).The sensitivity of diagnosing knee meniscus and articular cartilage injury and the ability of eliminating knee meniscus and articular cartilage injury in MR group were better than those in control group(P<0.05).Conclusion:In the diagnosis of knee meniscus and articular cartilage injury,CT is superior to MRI in the time of diagnosis,the rate of misdiagnosis and missed diagnosis,and the diagnostic efficiency.MRI has important diagnostic value in the diagnosis of knee meniscus and articular cartilage injury.
