摘 要:目的:探讨在机械通气患者中应用持续测压与间断测压,对预防呼吸机相关性肺炎(VAP)的效果。方法:选择本院收治的94例机械通气患者,纳入时间为2022年1月~2023年12月,按照住院时间分为对照组(2022年1月~12月)47例采用持续测压法,观察组47例(2023年1月~12月)采用间断测压法,观察两组机械通气时间、ICU住院时间、炎性指标[降钙素原(PCT)、C反应蛋白(CRP)、白细胞(WBC)]、VAP发生率。结果:两组机械通气时间、ICU住院时间比较无差异(P>0.05);与对照组比较,观察组VAP发生率明显更低(P<0.05)。干预前,两组PCT、CRP、WBC水平比较无差异(P>0.05);干预7d后,观察组PCT、CRP、WBC水平明显更低(P<0.05)。两组不良反应发生率比较无差异(P>0.05)。结论:与持续测压法相比,采用间断测压法能够有效减少机械通气患者发生VAP,减轻炎症反应,且安全性良好。Objective:To explore the effects of continuous and intermittent pressure measurement in the prevention of ventilator-associated pneumonia(VAP)in mechanically ventilated patients.Methods:94 mechanically ventilated patients admitted to our hospital were selected and included from January 2022 to December 2023.They were divided into a control group(January to December 2022)and an observation group(January to December 2023)of 47 patients using continuous pressure measurement.The observation group of 47 patients(January to December 2023)used intermittent pressure measurement to observe the mechanical ventilation time,ICU hospitalization time,inflammatory indicators[procalcitonin(PCT),C-reactive protein(CRP),white blood cells(WBC)],and incidence of VAP in both groups.There was no significant difference in mechanical ventilation time and ICU hospitalization time between the two groups(P>0.05);Compared with the control group,the incidence of VAP in the observation group was significantly lower(P<0.05).Before intervention,there was no significant difference in the levels of PCT,CRP,and WBC between the two groups(P>0.05);After 7 days of intervention,the levels of PCT,CRP,and WBC in the observation group were significantly lower(P<0.05).There was no difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Compared with continuous pressure measurement,intermittent pressure measurement can effectively reduce VAP in mechanically ventilated patients,alleviate inflammatory reactions,and has good safety.
