摘　　要：目的:观察高、低通量聚砜膜透析器对维持性血液透析(MHD)患者睡眠障碍相关临床指标的影响。方法:选取2021年1月~2022年12月天津市中医药研究院附属医院收治的62例MHD患者,回顾性分析患者相关临床资料,按照治疗方式不同分为对照组和观察组,各31例,对照组给予低通量聚砜膜透析器治疗,观察组给予高通量聚砜膜透析器治疗。观察两组治疗前后匹兹堡睡眠质量指数评分(PSQI)、尿素清除指数(Kt/V)、实验室指标[血清C-反应蛋白(CRP)、全段甲状旁腺激素(iPTH)、血红蛋白(Hb)、白蛋白(ALB)、钙(Ca)、磷(P)]的变化。结果:治疗后,两组PSQI评分下降,且观察组PSQI评分显著低于对照组(P<0.05);治疗后,观察组Kt/V数值升高,且观察组Kt/V数值显著高于对照组(P<0.05);治疗后,两组Hb水平升高,且观察组Hb水平显著高于对照组(P<0.05);治疗后,观察组CRP、iPTH及P水平下降,且观察组CRP、iPTH及P水平显著低于对照组(P<0.05)。结论:高通量聚砜膜透析器治疗通过有效清除中大分子毒素、缓解微炎症状态、提高透析充分性,可以明显改善MHD患者睡眠障碍。Objective:Observation of the effects of high and low flux polysulfone membrane dialyzers on clinical indicators related to sleep disorders in maintenance hemodialysis(MHD)patients.Methods:62 MHD patients admitted to the Affiliated Hospital of Tianjin Institute of Traditional Chinese Medicine from January 2021 to December 2022 were selected for retrospective analysis of their clinical data.They were divided into control group(n=31)and observation group(n=31).The control group was treated with a low flux polysulfone membrane dialyzer,while the observation group was treated with a high flux polysulfone membrane dialyzer.Observe the changes in pittsburgh sleep quality index(PSQI),urea clearance index(Kt/V),and laboratory indicators[serum C-reactive protein(CRP),intact parathyroid hormone(iPTH),hemoglobin(Hb),albumin(ALB),calcium(Ca),and phosphorus(P)]before and after treatment in two groups.Results:After treatment,the PSQI scores of two groups decreased compared to before treatment,and the PSQI score of the observation group after treatment was lower than that of the control group(P<0.05);The Kt/V value in the observation group increased after treatment compared to before treatment,and was higher than the same period level in the control group(P<0.05);After treatment,the Hb levels in two groups increased,and the Hb level in the observation group was higher than that in the control group(P<0.05);After treatment,the levels of CRP,iPTH,and P in the observation group decreased compared to the control group(P<0.05).Conclusion:High throughput polysulfone membrane dialyzer treatment can significantly improve sleep disorders in MHD patients by effectively clearing large and medium-sized toxins,alleviating micro inflammatory states,and improving dialysis adequacy.
