养元益肾散对透析低血压患者微炎症及营养状态的影响研究

摘　　要：目的 观察养元益肾散联合西医常规方法治疗血液透析低血压(intradialytic hypotension,IDH)患者炎症指标及营养状态的临床疗效和安全性。方法 研究对象为2022年9月—2024年1月河南中医药大学第一附属医院就诊的透析低血压患者60例,采用随机数字表法将患者分为对照组及治疗组(n=30)。两组均行血液透析治疗,对照组接受常规治疗,治疗组在对照组基础上加服养元益肾散进行治疗。治疗8周后,比较两组患者的临床疗效、主观综合性营养评分(subjective global assessment,SGA)、微炎症指标及外周血中营养指标。结果 治疗组的临床疗效总有效率达到93.33%,明显高于对照组的73.33%。两组之间差异有统计学意义(P<0.05)。治疗后,两组患者的SGA评分均有所升高,但加用中药的治疗组升高更为明显,差异有统计学意义(P<0.05)。治疗后两组患者的外周血中营养指标参数白蛋白(albumin,ALB)、前白蛋白(prealbumin,PAB)和血红蛋白(hemoglobin,Hb)水平均增高(P<0.05),且治疗组高于对照组,(P<0.05)。两组炎症因子高敏C-反应蛋白(high-sensitivity C-reactive protein,Hs-CRP)、白介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平降低(P<0.05),对照组更低,差异有统计学意义(P<0.05)。结论 养元益肾散能够改善血液透析低血压患者微炎症及营养状态,改善患者的临床表现,提升患者的透析效果和生活质量。Objective To observe the clinical efficacy and safety of Yangyuan Yishen powder combined with conventional western medicine in the treatment of patients with intradialytic hypotension(IDH),as well as its impact on their inflammation indexes and nutritional status.Methods Totally 60 patients with hemodialysis hypotension admitted to the First Affiliated Hospital of Henan University of Chinese Medicine from September 2022 to January 2024 were randomly divided into control group and treatment group(n=30) using the random number table method.Both groups received hemodialysis treatment,the control group received routine treatment,and the treatment group was treated with Yangyuan Yishen powder on the basis of the control group.After 8 weeks of treatment,the clinical efficacy,subjective global assessment(SGA),microinflammatory index and peripheral blood nutritional index of the two groups were evaluated.Results The treatment group exhibited a total effective rate of 93.33%,outperforming the control group,which recorded a rate of 73.33%.Additionally,the difference in the two groups was statistically significant(P<0.05).After treatment,the SGA scores of both groups were increased,but the increase was more obvious in the treatment group with Chinese medicine,and the difference was statistically significant(P<0.05).After treatment,the levels of albumin(ALB),prealbumin(PA),and hemoglobin(Hb) in the peripheral blood of both groups showed a significant increase(P<0.05).Notably,the treatment group exhibited higher levels of these parameters compared to the control group,and the differences were statistically significant(P<0.05).The levels of inflammatory factors,including high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),and tumor necrosis factor-α(TNF-α),exhibited a decrease in both groups(P<0.05).Notably,these levels were found to be lower in the control group compared to the treatment group,and the differences were statistically significant(P<0.05).Conclusion Yangyuan Yishen powder can improve microinflammati
