乳糖酶添加剂对新生儿乳糖不耐受的临床效果研究

摘　　要：目的探索乳糖酶添加剂对新生儿乳糖不耐受的临床效果。方法2022年7月至2023年12月对河南省人民医院新生儿科收治的乳糖不耐受新生儿进行研究,根据随机数字表法分为两组,各组50例,对照组完成47例,采用益生菌干预,研究组完成48例,在对照组基础上加用乳糖酶添加剂进行干预,干预2周后比较整体效果。结果两组干预后1周和2周身长增长、血清钙、血磷、碱性磷酸酶(alkaline phosphatase,AKP)差异无统计学意义(P>0.05),研究组干预后1周和2周的喂养量增加、喂养量、体重增长均高于对照组,同时,干预后1周和2周的吐奶、胃潴留、腹泻发生率均低于对照组,差异有统计学意义(P<0.05),研究组干预后1周粪便还原糖阳性率低于对照组,干预后2周粪便PH值阳性率、粪便还原糖阳性率低于对照组,差异有统计学意义(P<0.05)。结论对乳糖不耐受新生儿进行乳糖酶添加剂联合益生菌干预,可以更好减轻腹泻、吐奶、胃潴留等症状,改善胃肠道功能,促使健康生长发育。Objective To explore the clinical effect of lactase additive on neonatao lactose intolerance.Methods Lactose intolerant neonates admitted to the Department of Neonatology in the Henan Provincial People's Hospital from July 2022 to December 2023 were studied,and were divided into two groups according to the random number table method.There were 50 cases in each group.The control group completed 47 cases and received probiotic intervention,while the study group completed 48 cases and received lactase additive on the basis of the control group,and the overall effect was compared after 2 weeks of intervention.Results The two groups compared the length growth,serum calcium,blood phosphorus and alkaline phosphatase(AKP)at 1 and 2 weeks after intervention,and the differences were not statistically significant(P>0.05).The increase of feeding volume,feeding volume and weight gain at 1 week and 2 weeks after the intervention were higher than that of the control group,at the same time,the incidence of milk vomiting,gastric retention and diarrhea at 1 and 2 weeks after intervention were lower than that of the control group(P<0.05).The fecal reducing sugar positive rate of the study group was lower than that of the control group at 1 week after intervention,and the fecal PH positive rate and fecal reducing sugar positive rate of the study group were lower than that of the control group at 2 weeks after intervention(P<0.05).Conclusion Lacase additive combined with probiotic intervention for lactose intolerance neonates can better reduce diarrhea,vomiting,gastric retention and other symptoms,improve gastrointestinal function,and promote healthy growth and development.
