摘 要:目的分析呼出气一氧化氮(fractional exhaled nitric oxide,FENO)、脂氧素A4(lipoxin A4,LXA4)、白细胞介素5(interleukin-5,IL-5)、分泌型卷曲相关蛋白5(ssecreted frizzedrelated protein 5,SFRP5)在成人支气管哮喘诊断中的价值。方法选取2022年1月—2023年12月期间,河南省胸科医院135例急性发作期成人支气管哮喘患者作为哮喘组,按照疾病严重程度分为轻度组48例、中度组49例、重度组38例,并选取健康体检者135例作为对照组,比较哮喘组与对照组以及哮喘组患者不同严重程度的血清FENO、LXA4、IL-5、SFRP5,并分析其对成人支气管哮喘的诊断价值。结果与对照组比较,哮喘组FENO、LXA4、IL-5更高,哮喘组SFRP5更低,差异有统计学意义(P<0.05)。与轻度组比较,中度组、重度组FENO、LXA4、IL-5更高(P<0.05),中度组、重度组SFRP5更低(P<0.05),与中度组比较,重度组FENO、LXA4、IL-5更高(P<0.05)。四者联合=FENO+(-0.035/0.121)×LXA4+(0.104/0.121)×IL-5+(-0.709/0.121)×SFRP5,受试者工作特征(receiver operator characteristic curve,ROC)曲线分析显示,FENO、LXA4、IL-5、SFRP5、四者联合诊断成人支气管哮喘的曲线下面积(area under curve,AUC)分别为0.749、0.703、0.746、0.79、0.913,在最佳临界值对应的敏感度、特异度分别为:FENO为69.6%、68.1%,LXA4为56.3%、83.0%,IL-5为45.9%、100.0%,SFRP5为100.0%、49.6%,四者联合为85.9%、77.8%,四者联合诊断成人支气管哮喘的AUC高于FENO、LXA4、IL-5、SFRP5(P<0.05)。结论血清FENO、LXA4、IL-5、SFRP5联合对成人支气管哮喘具有较高的诊断价值。Objective To analyzed the value of fractional exhaled nitric oxide(FENO),lipoxin A4(LXA4),interleukin-5(IL-5),and ssecreted frizzedrelated protein 5(SFRP5)in the diagnosis of adult bronchial asthma.Methods From January 2022 to December 2023,135 adult patients with acute exacerbation of bronchial asthma in the Henan Chest Hospital were selected as the asthma group.They were divided into mild group(48 cases),moderate group(49 cases),and severe group(38 cases)according to the severity of the disease.Other 135 healthy individuals were selected as the control group.The serum levels of FENO,LXA4,IL-5,and SFRP5 were compared between the asthma group and the control group,as well as between the asthma group and patients with different severity levels.The diagnostic value of FENO,LXA4,IL-5,and SFRP5 for adult bronchial asthma was analyzed.Results Compared with the control group,the asthma group had higher levels of FENO,LXA4,and IL-5,while the asthma group had lower levels of SFRP5,and the differences were statistically significant(P<0.05).Compared with the mild group,the moderate and severe groups had higher levels of FENO,LXA4,and IL-5(P<0.05),while the moderate and severe groups had lower levels of SFRP5(P<0.05).Compared with the moderate group,the severe group had higher levels of FENO,LXA4,and IL-5(P<0.05).The combination of FENO+(-0.035/0.121)×LXA4+(0.104/0.121)×IL-5+(-0.709/0.121)×SFRP5.receiver operator characteristic(ROC)curve analysis showed that the AUC of FENO,LXA4,IL-5,SFRP5,and their combination in diagnosing adult bronchial asthma was 0.749,0.703,0.746,0.79,and 0.913,respectively.The sensitivity and specificity corresponding to the optimal threshold value were:FENO was 69.6%,68.1%,LXA4 was 56.3%,83.0%,IL-5 was 45.9%,100.0%,and SFRP5 was 100.0%,49.6%,respectively.The combination of the four was:77.8%,The AUC of combined diagnosis of adult bronchial asthma was higher than that of FENO LXA4,IL-5,SFRP5(P<0.05).Conclusion The combination of serum FENO,LXA4,IL-5,and SFRP5 has high diagnostic value for adult br
