处方前置审核助力PDCA模式下骨科中成药的合理应用探索

摘　　要：目的在处方前置审核的基础上,通过戴明环(plan-do-check-act,PDCA)循环,持续提高骨科中成药的合理用药水平。方法成立PDCA小组,从人员、机器设备、药品、方法和环境等方面入手,开展骨科中成药合理应用的探索。选取2023年1月—2023年6月省内2家医院(河南省胸科医院,河南省洛阳正骨医院)的骨科中成药处方作为对照组(活动前);选取2023年7月—2023年12月这2家医院的骨科中成药处方作为观察组(活动后),比较开展PDCA前后,处方前置审核检出的不合理用药情况及医务人员用药知识考核的得分情况。结果开展PDCA活动后,通过增加处方前置审核判定规则,完善中成药信息和数据库,骨科中成药处方的不合格率由9.87%降为2.20%,差异有统计学意义(P<0.05);医务人员的用药知识考核得分由(17.33±1.14)分提升到(21.75±1.16)分,差异有统计学意义(P<0.05)。结论PDCA管理可有效提高处方前置审核的合格率,促进合理用药,保证医疗安全。Objective To enhance the rational use of Chinese patent medicine in orthopedics by implementing the plan-do-check-act(PDCA)cycle on the basis of pre-prescription review.Methods A PDCA team was established to explore the rational application of Chinese patent medicine in orthopedics,focusing on personnel,equipment,medicines,methods,and environment.Prescriptions for Chinese patent medicine in orthopedics from two hospitals within the province(Luoyang Orthopedic-Traumatological Hospital of Henan Provincial,Henan Chest Hospital)from January to June 2023 were used as the control group(before the intervention);prescriptions from the same hospitals from July to December 2023 were used as the observation group(after the intervention).Compare the incidence of irrational drug use detected by pre-prescription review and the scores of medical staff's medication knowledge assessments before and after implementing PDCA.Results After the PDCA intervention,the unqualified rate of Chinese patent medicine prescriptions in orthopedics decreased from 9.87%to 2.20%,and the difference was statistically significant(P<0.05).The scores of medical staff's medication knowledge assessments improved from(17.33±1.14)to(21.75±1.16),and the difference was statistically significant(P<0.05).Conclusion PDCA management can effectively improve the qualified rate of pre-prescription review,promote rational drug use,and ensure medical safety.
