摘  要:创伤性低血压休克是创伤性死亡的主要原因,其救治成功率始终不令人满意。为提高救治有效率,需要探索现场救治新策略。针对救治现场难以建立有效输液通道的情况,有学者提出了允许性低血压概念。针对转运效率与现场救治的优先级别问题,强调现场救治要以减少伤员的病死率和致残率为目标,权衡现场救治的必要性与所需耗费的时间,尽快将患者转运到医院急诊室救治。技术方面,低强度脉冲超声的升血压作用可能为低血压休克患者的救治带来新突破。现阶段,创伤性低血压休克救治的技术还存在恶劣环境中难以开展,救治效率不足以及操作复杂等问题,提高运送效率仍是提高救治效率的重要措施之一。Traumatic hypotensive shock is a critical and primary cause of traumatic death,yet the efficiency of treatment remains unsatisfactory.New concepts and technologies are continually being proposed.In view of the difficulty to establish effective venous access on the scene,leading to the concept of permissive hypotension.The blood pressure-raising effect of low-intensity pulsed ultrasound proposed by scholars in China might bring about a new breakthrough.However,at this stage,treatment technologies for traumatic hypotensive shock still face challenges such as the difficulty in conducting treatments in severe injury scenarios,insufficient advancement,and difficulties in implementation.Improving transportation efficiency remains one of the important measures.
