血清微小RNA-378、微小RNA-215-5p与活动性肺结核患儿病情严重程度及预后的相关性分析

摘　　要：目的探究活动性肺结核患儿血清微小RNA-378(microRNA-378,miR-378)、微小RNA-215-5p(microRNA-215-5p,miR-215-5p)水平变化,并分析其与病情严重程度、预后的关系。方法选取中国人民解放军总医院第七医学中心2022年1月至2023年7月收治的102例活动性肺结核患儿作为活动性肺结核组,选取同期收治的102例非活动性肺结核患儿作为非活动性肺结核组,选取健康体检儿童102例为对照组。按照活动性肺结核患儿病情严重程度分为轻症组(58例)和重症组(44例);根据患儿预后情况分为预后良好组(64例)和预后不良组(38例)。采用血清miR-378、miR-215-5p水平采用实时荧光定量-聚合酶链反应(quantitative real-time PCR,qRT-PCR)检测;Spearman法对活动性肺结核患儿血清miR-378、miR-215-5p与病情严重程度及预后进行相关性分析;活动性肺结核患儿预后的影响因素采用Logistic回归分析。结果活动性肺结核组血清miR-378、miR-215-5p水平高于非活动性肺结核组、对照组(P<0.05),且非活动性肺结核组血清miR-378、miR-215-5p水平高于对照组(P<0.05)。重症组血清miR-378、miR-215-5p水平高于轻症组(P<0.05)。预后不良组血清miR-378、miR-215-5p水平高于预后良好组(P<0.05)。血清miR-378、miR-215-5p与活动性肺结核患儿病情严重程度及预后不良呈正相关(P<0.05)。多因素Logistic回归分析结果显示,miR-378高表达、miR-215-5p高表达、病情严重程度为重症是活动性肺结核患儿预后不良的危险因素(P<0.05)。结论活动性肺结核患儿血清miR-378、miR-215-5p水平升高,与病情严重程度及预后不良呈正相关。Objective To explore the changes of serum MicroRNA-378(miR-378),MicroRNA-215-5p(miR-215-5p)levels in children with active pulmonary tuberculosis,and analyze their relationship with the severity and prognosis of the disease.Method 102 children with active pulmonary tuberculosis admitted to the Seventh Medical Center of Chinese People's Liberation Army General Hospital from January 2022 to July 2023 were selected as the active pulmonary tuberculosis group,102 children with inactive pulmonary tuberculosis were selected as the inactive pulmonary tuberculosis group,and 102 children with healthy physical examination were selected as the control group.According to the severity of active pulmonary tuberculosis in children,they were divided into mild group(58 cases)and severe group(44 cases);According to the prognosis of the children,they were divided into a good prognosis group(64 cases)and a poor prognosis group(38 cases).Serum miR-378 and miR-215-5p levels were detected by real-time fluorescence quantitative polymerase chain reaction(qRT-PCR).Spearman method was applied to analyze the correlation between serum miR-378 and miR-215-5p with disease severity and prognosis in children with active pulmonary tuberculosis.Logistic regression analysis was used to analyze the prognostic factors of children with active pulmonary tuberculosis.Result The serum levels of miR-378 and miR-215-5p in active pulmonary tuberculosis group were higher than those in inactive pulmonary tuberculosis group and control group(P<0.05),and the serum levels of miR-378 and miR-215-5p in inactive pulmonary tuberculosis group were higher than those in control group(P<0.05).The serum levels of miR-378 and miR-215-5p in severe group were higher than those in mild group(P<0.05).The serum levels of miR-378 and miR-215-5p in poor prognosis group were higher than those in good prognosis group(P<0.05).Serum miR-378 and miR-215-5p are positively correlated with the severity of illness and poor prognosis in children with active pulmonary tuberculosis(P<0.05).Mu
