摘  要:目的探讨目标体温管理(Targeted Temperature Management,TTM)在严重颅脑损伤(Traumatic Brain Injury,TBI)合并创伤性凝血病(Trauma-Induced Coagulopathy,TIC)患者中的治疗效果。方法选取2023年6月至2024年6月无锡市惠山区人民医院收治的46例严重颅脑损伤合并TIC患者,随机分为对照组和观察组,对照组进行常规体温管理,观察组进行TTM。结果观察组在神经功能评分(GCS和NIHSS)、颅内压和脑血流量和疗效评估(GOS评分)均优于对照组(P<0.001),且观察组的并发症发生率低于对照组(P<0.05)。结论TTM能够改善严重TBI合并TIC患者的神经功能评分、降低颅内压和脑血流量、减少并发症发生率、改善预后,应被视为此类患者治疗中的一个重要组成部分。Objective To investigate the therapeutic effects of Targeted Temperature Management(TTM)in patients with severe traumatic brain injury(TBI)complicated by trauma-induced coagulopathy(TIC).Methods A total of 46 patients with severe TBI and TIC admitted to Huishan District People’s Hospital in Wuxi from June 2023 to June 2024 were selected and randomly divided into a control group and an observation group.The control group received standard temperature management,while the observation group underwent TTM.Results The observation group demonstrated significantly better outcomes in neurological function scores(GCS and NIHSS),intracranial pressure,cerebral blood flow,and efficacy assessments(GOS scores)compared to the control group(P<0.001).Additionally,the incidence of complications in the observation group was lower than that in the control group(P<0.05),with all differences being statistically significant.Conclusion TTM can improve neurological function scores,reduce intracranial pressure and cerebral blood flow,and decrease complication rates in patients with severe TBI complicated by TIC,thereby enhancing prognosis.It should be considered an important component in the treatment of this patient population.
