一例ECMO联合CRRT在肺出血-肾炎综合征患者救治中的临床应用报告

摘　　要：目的 总结体外膜肺氧合(extracorporeal membrane oxygenation, ECMO)联合连续性肾脏替代治疗(continuous renal replacement therapy, CRRT)在肺出血-肾炎综合征患者救治中的临床护理经验。方法 观察ECMO联合CRRT在救治过程中的运行护理、抗凝治疗、ECMO相关下肢缺血坏死护理效果等。结果 患者入院28d后转出ICU,住院77d后好转出院。结论 在医护团队合作模式下,对本例肺出血-肾炎综合征患者快速启动ECMO支持联合CRRT等综合治疗措施,救治成功经验值得临床借鉴。Objective To summarize the nursing of a case of Goodpasture Syndrome treated by extracorporeal membrane oxygenation combined with continuous renal replacement therapy.Methods The operation nursing of ECMO combined with CRRT,Anticoagulant Therapy,ECMO related lower limb ischemia and necrosis nursing.Results The patient was transferred from ICU 25days after admission and discharged after 77days of hospitalization.Conclusions Under the medical team cooperation mode,ECMO support combined with CRRT and other comprehensive treatment and nursing measures were quickly started for this patient with Goodpasture Syndrome.The treatment was successful,which is worthy of clinical reference.
