摘 要:目的研究老年谵妄患者血浆Aβ和Tau蛋白水平与谵妄的相关性。方法选取2020年11月至2022年11月四川大学华西医院老年科96例患者,采用单分子免疫阵列(Simoa)测量血浆中Tau蛋白、Aβ40和Aβ42含量,比较谵妄发病组(32例)、谵妄好转组(32例)和对照组(32例)间生物标志物水平,并分析神经损伤标志物水平对谵妄的诊断价值。结果谵妄发病组的血浆Tau蛋白、Aβ40、Aβ42水平高于对照组(Tau蛋白:P=0.01,Aβ40:P=0.001,Aβ42:P<0.001),谵妄好转组血浆Tau蛋白、Aβ40和Aβ42水平与谵妄发病组差异无统计学意义(Tau蛋白:P=0.256,Aβ40:P=0.834,Aβ42:P=0.913)。ROC分析结果显示,谵妄期间血浆Tau蛋白和Aβ40、Aβ42水平对谵妄均有诊断价值。结论血浆Aβ和Tau蛋白的水平变化有助于早期判断谵妄,同时为研究谵妄的发病机制和谵妄生物标志物提供理论依据。Objective To study the dynamic changes in plasma Aβand Tau protein levels in geriatric delirium patients and their correlation with delirium and to provide theoretical support for the pathogenesis of delirium.Methods A total of 96 patients from the department of geriatrics at West China Hospital of Sichuan University were included from November 2020 to November 2022.Single molecule immune array(Simoa)was used to measure the levels of Tau protein,Aβ40,and Aβ42 in plasma.The levels of biomarkers were compared among the delirium onset group(n=32),delirium improvement group(n=32),and control group(n=32),and the diagnostic value of neural injury markers for delirium was analyzed.Results Plasma Tau protein,Aβ40,and Aβ42 levels during the onset of delirium were significantly elevated compared with those in the control group(Tau:P=0.01,Aβ40:P=0.001,Aβ42:P<0.001).There was no statistically significant difference in the levels of plasma Tau protein,Aβ40,and Aβ42 between the delirium improvement group and the delirium onset group(Tau:P=0.256,Aβ40:P=0.834,Aβ42:P=0.913).The results of ROC analysis showed that plasma tau protein and Aβ40,Aβ42 levels had diagnostic value during delirium.Conclusions The changes in blood levels of Aβand Tau protein has a certain guiding role in the early diagnosis of delirium and simultaneously provides a certain theoretical basis for the study of the pathogenesis of delirium and delirium biomarkers.
