摘 要:目的 探究关节外科住院医师规范化培训教学中开展教师模拟标准化病人(teather as standardized patient, TSP)联合情景设计视频模式的教学实施效果。方法 选取2022年7月-2023年6月山东第二医科大学附属医院关节外科70名住院医师为研究对象,其中2022年7-12月的35名住院医师接受传统教学模式,作为对照组;2023年1-6月的35名住院医师接受TSP联合情景设计视频模式,作为观察组;对比两组住院医师的考核成绩,利用改良版迷你临床演练评估量表(Mini-Clinical Evaluation Exercise, Mini-CEX)、评判性思维能力(Critical Thinking Disposition Inventory-Chinese Version, CTDI-CV)调查住院医师的临床实践能力和评判性思维能力。结果 观察组医师的理论考核、实践操作考核成绩分别为(93.69±4.33)分、(92.14±5.36)分,均高于对照组的(83.63±4.87)分、(84.41±6.58)分,差异有统计学意义(t=9.133、5.389,P均<0.05)。培训后两组Mini-CEX、CTDI-CV评分均较培训前升高,且观察组优于对照组,差异有统计学意义(P均<0.05)。结论 关节外科住院医师规范化培训教学中实行TSP联合情景设计视频模式,具有显著教学效果,可提高住院医师的理论知识掌握和专业技能,并发展综合能力和评判性思维能力。Objective To explore the teaching effect of joint scenario design video mode of teacher as standardized pa-tient(TSP)in standardized training teaching of joint surgery residents.Methods Seventy residents of the Department of Joint Surgery in the Affiliated Hospital of Shandong Second Medical University from July 2022 to June 2023 were selected as the study objects,among which thirty-five residents from July to December 2022 accepted the traditional teaching mode as the control group;From January to June 2023,thirty-five residents received TSP combined scenario design video mode as the observation group;Comparing the scores of the residents in the two groups,Using the modi-fied Mini-Clinical Evaluation Exercise(Mini-CEX),and Critical Thinking Disposition Inventory-Chinese Version(CTDI-CV)were used to investigate the clinical practice ability and critical thinking ability of residents.Results The scores of theoretical assessment and practical operation assessment in the observation group were 93.69±4.33 and 92.14±5.36,which were higher than 83.63±4.87 and 84.41±6.58 of the control group,the differences were statistically significant(t=9.133,5.389,all P<0.05).After training,the Mini-CEX and CTDI-CV scores of the two groups were higher than those before training,and the observation group were better than the control group,and the differences were statistically significant(all P<0.05).Conclusion The implementation of TSP combined scene design video mode in the standardized training and teaching of joint surgery residents has significant teaching effects,improves the theo-retical knowledge and professional skills of residents,and develops the comprehensive ability.
