老年轻度认知障碍患者焦虑的潜在剖面分析及其影响因素研究

摘　　要：目的探讨老年轻度认知障碍患者焦虑的潜在类别及不同类别的影响因素。方法采用一般资料调查表、汉密尔顿焦虑量表、蒙特利亚认知评估量表对江西省某三级甲等医院神经内科283例患者进行调查,采用潜在剖面分析法和多元Logistic回归分析探讨老年轻度认知障碍患者焦虑的潜在类别及其影响因素。结果老年轻度认知障碍患者焦虑可以分为3个类别,分别命名为“低水平焦虑-认知障碍组”(40.99%)、“中水平精神性焦虑组”(40.28%)、“高水平精神性焦虑组”(18.73%)。Logistic回归分析结果显示,慢性病种类、自评听力下降、认知功能是老年轻度认知障碍患者焦虑的影响因素(P<0.05)。结论老年轻度认知障碍患者焦虑存在异质性,建议医务人员根据患者焦虑的不同类别实施精准干预。Objective To explore the potential categories and different categories of influencing factors of anxiety symptoms in elderly patients with mild cognitive impairment(MCI).Methods The general data questionnaire,Hamilton Anxiety Scale and Monteria Cognitive Assessment Scale were used to conduct a cross-sectional investigation among 283 patients in a tertiary Class A hospital in Jiangxi Province.Latent profile analysis and multinomial Logistic regression analysis were used to explore the potential categories and influencing factors of anxiety in elderly patients with MCI.Results The anxiety symptoms of elderly patients with MCI could be divided into three categories:low anxiety-cognitive disorder group(40.99%),medium mental anxiety group(40.28%)and high mental anxiety group(18.73%).Logistic regression analysis showed that the number of chronic diseases,self-rated hearing loss and cognitive function were the influencing factors of MCl in the elderly(P<0.05).Conclusion There is heterogeneity of anxiety symptoms in elderly patients with MCI.It is suggested that we should implement targeted intervention according to different categories of anxiety.
