摘 要:目的探究维生素D通过抗苗勒氏管激素(AMH)调节Smad信号通路对少、弱精子症患者精液质量的影响。方法回顾性抽取2021年7月至2022年12月于郑州大学附属洛阳中心医院生殖医学中心就诊的50例少、弱精子症患者作为研究对象。所有患者均采用肌肉注射维生素D2、口服碳酸钙D3片及日光照射进行干预。干预前后,采用手淫法收集精液,采用精子微生物动/静态图像检测分析仪对精液进行分析,采用Diff-Quick法行精子形态学检查,采用酶联免疫吸附试验法检测精浆AMH、Smad2、Smad7及血清25-羟维生素D[25(OH)D]水平。结果干预后,50例少、弱精子症患者的精液量、密度、A级精子率、精子成活率、前向运动精子占比、精子浓度及血清25(OH)D、精浆AMH、精浆Smad2指标均高于干预前,精浆Smad7水平低于干预前(P<0.05)。Pearson相关分析结果显示,血清25(OH)D与精浆AMH、Smad2均呈正相关性(R=0.36、0.29,P均<0.001),与Smad7呈负相关性(R=-0.45,P<0.001)。结论维生素D对于男性精液质量具有一定的调节作用,其作用机制可能与通过AMH调节Smad信号通路有关。ObjectiveTo investigate the effect of vitamin D on semen quality by regulating the Smad signaling pathway through anti-Müllerian hormone(AMH)in patients with oligozoospermia and asthenozoospermia.MethodsFifty patients with oligozoospermia and asthenozoospermia who visited the Reproductive Medicine Center of Luoyang Central Hospital Affiliated to Zhengzhou University from July 2021 to December 2022 were retrospectively selected as object of study.All patients were intervened with intramuscular injection of vitamin D2,oral administration of calcium carbonate D3 tablets,and exposure to sunlight.Before and after intervention,semen was collected using masturbation method detection system,and the semen was analyed by computer-assisted sperm analysis.Sperm morphology was examined using Diff-Quick method,and seminal plasma levels of AMH,Smad2,Smad7,and serum 25 hydroxyvitamin D,also known as 25(OH)D,were detected using enzyme-linked immunosorbent assay.ResultsAfter intervention,the semen volume,density,A-grade sperm rate,sperm survival rate,forward motile sperm,sperm concentration,serum 25(OH)D level,seminal plasma AMH,and seminal plasma Smad2 of the 50 patients with oligozoospermia and asthenozoospermia were all higher than those before intervention,and seminal plasma Smad7 level was lower than that before intervention(P<0.05).Pearson correlation analysis results revealed that serum 25(OH)D was positively correlated with seminal plasma AMH and Smad2(R=0.36,0.29;both P<0.001),and it was negatively correlated with seminal plasma Smad7(R=-0.45,P<0.001).ConclusionsVitamin D has a certain regulatory effect on male semen quality,and the mechanism may be related to regulating Smad signaling pathway through AMH.
