INFIX技术与外固定支架分别联合骶髂关节螺钉治疗Tile B型骨盆骨折的效果比较

摘　　要：目的比较前环皮下内置外固定架(INFIX)技术与外固定支架分别联合骶髂关节螺钉(SJS)治疗Tile B型骨盆骨折的临床效果。方法队列研究。抽取南阳市第二人民医院2018年1月至2024年4月收治的Tile B型骨盆骨折患者64例,按随机数字表法分为INFIX组和外支架组,每组32例。INFIX组采用INFIX技术联合SJS治疗,外支架组采用外固定支架联合SJS治疗。比较两组手术指标(手术时间、术中出血量、住院天数、骨折愈合时间)、术后疼痛程度[视觉模拟评分表(VAS)评分]、骨盆功能[Majeed骨盆功能评分(MPS)、欧洲五维健康量表(EQ-5D)]及并发症发生率。结果INFIX组住院天数、骨折愈合时间短于外支架组(P<0.05)。术后1、3、6个月,INFIX组VAS评分低于外支架组(P<0.05)。术后3、6个月,INFIX组术MPS、EQ-5D评分较高于外支架组(P<0.05)。INFIX组并发症发生率(6.25%,2/32)低于外支架组(25.00%,8/32),P<0.05。结论与外固定支架相比,INFIX技术联合SJS治疗Tile B型骨盆骨折的效果更好,能减轻机体疼痛,促进骨折愈合,缩短住院天数,且该方案并发症较少。ObjectiveTo compare the effects of anterior subcutaneous internal fixation(INFIX)technique and external fixator in combined therapy with sacroiliac joint screw(SJS)for Tile B type pelvic fractures.MethodsA total of 64 patients with Tile B type pelvic fractures treated in Nanyang Second General Hospital from January 2018 to April 2024 were selected for the cohort study.And they were divided into the INFIX group and the external fixator group according to the random number table method,with 32 cases in each group.The INFIX group was treated by INFIX technique combined with SJS,and the external fixator group was treated by external fixator combined with SJS.The surgical indicators(operation duration,intraoperative blood loss,hospital stay and fracture healing time),postoperative pain evaluated by visual analogue scale(VAS)score,pelvic function assessed by Majeed pelvic function score(MPS)and the European five-dimensional health scale(EQ-5D)score,and the incidence of complications of the two groups were compared.ResultsCompared with the external fixator group,the hospital stay and fracture healing time of the INFIX group were shorter(P<0.05).Compared with the external fixator group,the INFIX group had lower VAS scores 1,3 and 6 months after surgery(P<0.05).Compared with the external fixator group,the INFIX group had higher MPS and EQ-5D scores 3 and 6 months after surgery(P<0.05).The incidence of complications in the INFIX group was 6.25%(2/32),lower than the 25.00%(8/32)in the external fixator group(P<0.05).ConclusionsCompared with the external fixator,the INFIX technique combined with SJS has better effects in the treatment of Tile B type pelvic fractures,which can alleviate the body pain,promote fracture healing,shorten the hospital stay,and decrease the incidences of complications.
