摘 要:目的探讨心肌超声造影(MCE)联合三维斑点追踪成像(3D-STI)评估心肌缺血的价值。方法回顾性抽取2019年6月至2023年6月商丘市第一人民医院收治的65例疑似发生心肌缺血的冠心病患者为研究对象,以冠状动脉造影结果为"金标准",将65例冠心病患者分为心肌缺血组和非心肌缺血组,对比两组MCE、3D-STI结果,通过受试者工作特征曲线探讨MCE联合3D-STI评估心肌缺血的价值。结果经冠状动脉造影检查,65例疑似心肌缺血的冠心病患者中诊断出心肌缺血47例,占72.31%。心肌缺血组整体长轴应变、整体圆周应变、整体面积应变水平高于非心肌缺血组,整体径向应变、心肌血容量、心肌平均血流速度、心肌血流量均低于非心肌缺血组(P均<0.05)。绘制受试者工作特征曲线结果显示,MCE联合3D-STI评估心肌缺血的曲线下面积值为0.896,具有一定评估价值。结论MCE联合3D-STI可较好评估心肌缺血价值较高,可为临床心肌缺血的快速诊断提供依据。ObjectiveTo investigate the value of myocardial contrast echocardiography(MCE)combined with three-dimensional speckle-tracking imaging(3D-STI)in evaluating myocardial ischemia.MethodsSixty-five suspected coronary heart disease patients admitted to the First People’s Hospital of Shangqiu from June 2019 to June 2023 were retrospectively selected as the research subjects.Using coronary angiography results as the gold standard,65 patients identified with coronary heart disease were divided into the myocardial ischemia group and the non myocardial ischemia group.The results of MCE and 3D-STI were compared between the two groups.And the value of MCE combined with 3D-STI in evaluating myocardial ischemia was assessed by receiver operating characteristic curve.ResultsAccording to coronary angiography examination,47 of 65 suspected coronary heart disease patients were diagnosed with myocardial ischemia,accounting for 72.31%.The levels of global longitudinal strain,global circumferential strain and global area strain in the myocardial ischemia group were higher than those in the non-myocardial ischemia group,while the global radial strain,myocardial blood volume,myocardial mean blood flow velocity and myocardial blood flow were lower than those in the non myocardial ischemia group(all P<0.05).The results of receiver operating characteristic curve showed that the area under the curve of MCE combined with 3D-STI for evaluating myocardial ischemia was 0.896,suggesting certain evaluation value.ConclusionsThe combination of MCE and 3D-STI can effectively evaluate myocardial ischemia,so as to provide a basis for rapid diagnosis of clinical myocardial ischemia.
