摘 要:目的探讨心电图P波电位对心房颤动(以下简称房颤)患者射频消融术(RFCA)后复发的预测价值。方法前瞻性抽取2020年1月至2022年12月于河南科技大学第一附属医院拟行RFCA的102例房颤患者为研究对象,于RFCA前测量心电图P波电位参数,RFCA后对所有患者随访3个月,依据短期复发情况将其分为复发组与未复发组,对比两组患者RFCA前心电图P波电位参数,采用点二列相关性分析心电图P波电位参数与房颤患者RFCA后复发的关系,并绘制受试者工作特征曲线,计算曲线下面积(AUC)评估心电图P波电位参数对房颤患者RFCA后复发的预测价值。结果102例患者中1例放弃治疗,1例治疗失败,1例失访,共有99例患者完成治疗及随访,其中27例患者在3个月内复发,复发率为27.27%。复发组RFCA前心电图P波最大时限(Pmax)、P波离散度(Pd)均大于未复发组,P波振幅(Pa)小于未复发组,差异有统计学意义(P<0.05)。点二列相关性分析显示,Pmax、Pd水平均与房颤患者RFCA后复发风险呈正相关(r=0.44、0.44,P均<0.05),Pa水平与房颤患者RFCA后复发风险呈负相关(r=-0.34,P<0.05)。绘制受试者工作特征曲线结果显示,Pmax、Pa、Pd单项及联合预测房颤患者RFCA后复发的AUC分别为0.767、0.706、0.751、0.885。结论心电图P波电位参数Pmax、Pa、Pd对房颤患者RFCA后复发均具有一定预测价值,且三者联合预测价值更高。ObjectiveTo investigate the predictive value of electrocardiogram P-wave potential for recurrence of atrial fibrillation after radiofrequency ablation(RFCA).MethodsOne hundred and two patients with atrial fibrillation who was going to have RFCA in the First Affiliated Hospital of Henan University of Science and Technology from January 2020 to December 2022 were prospectively selected as the study subjects.The P-wave potential parameters on electrocardiogram of all selected patients were measured before RFCA.All patients were followed up for 3 months after RFCA.According to the short-term recurrence,they were divided into the recurrence group and the non recurrence group.The P-wave potential parameters on electrocardiogram before RFCA were compared between the two groups.The relationship between P-wave potential parameters on electrocardiogram and recurrence after RFCA in patients with atrial fibrillation was analyzed by point-biserail correlation analysis;and the receiver operating characteristic curve was drawn to calculate the area under the curve(AUC),so as to evaluate the predictive value of P-wave potential parameters on electrocardiogram for recurrence of atrial fibrillation after RFCA.ResultsAmong the 102 patients,1 patient gave up treatment,1 case failed treatment,and 1 case was lost to follow-up.A total of 99 patients completed the treatment and follow-up,of which 27 patients relapsed within 3 months,with a recurrence rate of 27.27%.The maximum P-wave duration(Pmax)and P-wave dispersion(Pd)of the electrocardiogram before RFCA in the recurrence group were greater than those in the non recurrence group,and the P-wave amplitude(Pa)was smaller than that in the non recurrence group(P<0.05).Point-biserail correlation analysis revealed that both Pmax and Pd levels were positively correlated with the risk of recurrence of atrial fibrillation after RFCA(r=0.44,0.44;both P<0.05),while Pa level was negatively correlated with the risk of recurrence of atrial fibrillation after RFCA(r=-0.34,P<0.05).The results of rec
