消瘿汤加减联合优甲乐治疗桥本氏甲状腺炎合并甲状腺结节的效果观察

摘　　要：目的观察消瘿汤加减联合优甲乐治疗桥本氏甲状腺炎合并甲状腺结节的临床疗效。方法队列研究。抽取2020年5月至2023年10月郑州大学第二附属医院收治的桥本氏甲状腺炎合并甲状腺结节患者62例,采用随机数字表法分为对照组和研究组,每组31例。对照组采用优甲乐治疗,研究组在对照组的基础上加服消瘿汤。比较两组临床疗效、甲状腺抗体[甲状腺球蛋白抗体(TGAb)、甲状腺过氧化物酶抗体(TPOAb)]水平、炎症因子[白细胞介素-10(IL-10),白细胞介素-6(IL-6)、γ-干扰素(IFN-γ)]水平及甲状腺情况。结果治疗后,研究组总有效率(93.55%,29/31)高于对照组(67.74%,21/31),P<0.05。治疗后,两组TGAb、TPOAb水平低于治疗前,且研究组低于对照组(P<0.05)。治疗后,两组IL-10水平高于治疗前,IL-6及IFN-γ水平低于治疗前(P<0.05),且研究组IL-10水平高于对照组,IL-6及IFN-γ水平低于对照组(P<0.05)。治疗后,两组甲状腺结节直径、甲状腺体积均小于治疗前,且研究组小于对照组(P<0.05)。结论消瘿汤加减联合优甲乐能有效提高桥本氏甲状腺炎合并甲状腺结节的临床疗效,降低炎症因子水平,调节机体免疫,改善临床症状,对疾病的康复有重要意义。ObjectiveTo investigate the efficacy of Xiaoying decoction combined with euthyrox in the treatment of Hashimoto thyroiditis complicated by thyroid nodules.MethodsA total of 62 patients with Hashimoto thyroiditis complicated by thyroid nodules admitted to the Second Affiliated Hospital of Zhengzhou University from May 2020 to October 2023 were selected for the cohort study,and they were divided into the control group and the study group by random number table method,with 31 cases in each group.The control group was treated with euthyrox,and the study group was treated with Xiaoying decoction based on the treatment of the control group.The clinical efficacy,levels of thyroid antibodies including thyroglobulin antibody(TGAb)and thyroid peroxidase antibody(TPOAb),levels of inflammatory factors,including interleukin-10(IL-10),interleukin-6(IL-6)andγ-interferon(IFN-γ),and the thyroid status were compared between the two groups.ResultsAfter treatment,the total effective rate of the study group was 93.55%(29/31),higher than the 67.74%(21/31)of the control group(P<0.05).After treatment,the levels of TGAb and TPOAb in the two groups were lower than those before treatment,and the study group had lower TGAb and TPOAb than the control group(P<0.05).After treatment,the levels of IL-10 in the two groups were higher than those before treatment,and the levels of IL-6 and IFN-γwere lower than those before treatment(P<0.05).Compared with the control group,the study group had higher IL-10 level,lower IL-6 level and lower IFN-γlevel after treatment(P<0.05).After treatment,the diameter of thyroid nodules and thyroid volume in the two groups were smaller than those before treatment,and these indexes in the study group were smaller compared with the control group(P<0.05).ConclusionsThe combination of modified Xiaoying decoction and euthyrox can effectively improve the clinical efficacy of Hashimoto thyroiditis with thyroid nodules,reduce the level of inflammatory factors,regulate the body’s immunity,and alleviate clinical symptoms
