磷酸肌酸钠联合达格列净片治疗2型糖尿病合并心力衰竭的效果观察

摘　　要：目的探讨磷酸肌酸钠联合达格列净片治疗2型糖尿病(T2DM)合并心力衰竭的临床效果。方法队列研究。抽取2021年10月至2023年10月神木市医院收治的T2DM合并心力衰竭患者104例为研究对象,按信封法分为对照组和观察组,每组52例。对照组采用常规方法干预,观察组联合磷酸肌酸钠和达格列净片治疗,两组均治疗4周。比较两组血糖水平、心功能[心脏指数(CI)、左心室做功指数(LVSWI)、右心室做功指数(RVSWI)]、血清N末端脑钠肽前体(NT-proBNP)及药物安全性。结果干预3个月后,两组血糖水平均降低,且观察组血糖水平低于对照组(P<0.05)。干预后,两组心功能均得到改善,CI高于干预前,LVSWI、RVSWI、NT-proBNP水平低于干预前(P<0.05),且观察组CI、LVSWI、RVSWI高于对照组,NT-proBNP水平低于对照组(P<0.05)。两组药物安全性比较差异未见统计学意义(P>0.05)。结论磷酸肌酸钠联合达格列净片能降低T2DM合并心力衰竭患者血糖水平,有助于提高患者心功能,降低NT-proBNP水平,且药物安全性较高。Objective To investigate the therapeutic effect of creatine phosphate sodium combined with dapagliflozin tablets on patients with type 2 diabetes mellitus(T2DM)complicated by heart failure.Methods A total of 104 patients with T2DM complicated by heart failure admitted to Shenmu Hospital from October 2021 to October 2023 were selected as the research objects for the cohort study,and they were divided into the control group and the observation group by envelope method,with 52 cases in each group.The control group was intervened by conventional methods,and the observation group was treated with creatine phosphate sodium and dapagliflozin tablets,both groups were treated for 4 weeks.The blood glucose level,cardiac function assessed by cardiac index(CI),left ventricular stroke work index(LVSWI)and right ventricular stroke work index(RVSWI),serum N-terminal pro-brain natriuretic peptide(NT-proBNP)and drug safety were compared between the two groups.ResultsAfter 3 months of intervention,the blood glucose levels of the two groups were lower than those before treatment,and the blood glucose level of the observation group was lower than that of the control group(P<0.05).After intervention,the cardiac function of the two groups was improved.The CI,LVSWI and RVSWI of the observation group were higher than those of the control group,and the level of NT-proBNP was lower than that of the control group(P<0.05).There was no significant difference in drug safety between the two groups(P>0.05).Conclusions Creatine phosphate sodium combined with dapagliflozin tablets can reduce the blood glucose level of patients with T2DM complicated by heart failure,help to improve the cardiac function of patients,and reduce the level of NT-proBNP,with high drug safety.
