温经汤对反复自然流产患者子宫内膜容受性的影响

摘　　要：目的观察使用温经汤治疗反复自然流产(recurrent spontaneous abortion,RSA)的疗效,及其对子宫内膜容受性的影响。方法选取2022年2月至2024年2月河南省中医院(河南中医药大学第二附属医院)收治的89例RSA患者作为研究对象,随机分成温经汤联合治疗组(观察组,n=45)和常规治疗组(对照组,n=44)。比较中医症状积分、疗效及子宫内膜容受性[子宫内膜厚度、搏动指数(pulsation index,PI)、阻力指数(resistance index,RI)]、性激素水平[孕酮(progesterone,P)、雌二醇(estradiol,E2)、绒毛膜促性腺激素(human chorionic gonadotropin,HCG)]、免疫功能(CD3+、CD4+、CD4+/CD8+)。结果两组RSA患者治疗后中医症状积分低于治疗前(P<0.05);治疗后观察组中医症状积分低于对照组(P<0.05)。观察组临床有效率95.56%高于对照组的79.55%(P<0.05)。两组RSA患者治疗后子宫内膜厚度均高于治疗前(P<0.05),PI、RI均低于治疗前(P<0.05);治疗后观察组子宫内膜厚度高于对照组(P<0.05),PI、RI均低于对照组(P<0.05)。两组RSA患者治疗后P、E2、HCG水平均高于治疗前(P<0.05);治疗后观察组P、E2、HCG水平均高于对照组(P<0.05)。两组RSA患者治疗后CD3+、CD4+、CD4+/CD8+水平均高于治疗前(P<0.05);治疗后观察组CD3+、CD4+、CD4+/CD8+水平均高于对照组(P<0.05)。结论温经汤可提升RSA患者的临床治疗效果,并改善子宫内膜容受性。Objective To observe the effect of Wenjing Decoction on recurrent spontaneous abortion and its influence on endo⁃metrial receptivity.Methods Prospective,89 patients with recurrent spontaneous abortion admitted to Henan Provincial Hospital of Traditional Chinese Medicine(Second Affiliated Hospital of Henan University of Traditional Chinese Medicine)from February 2022 to February 2024 were selected as the research.They were randomly divided them into two groups using the random number table:conven⁃tional Western medicine treatment group(control group,n=44)and the combined treatment group with Wenjing decoction(observa⁃tion group,n=45).The Chinese medicine symptom score and curative effect,endometrial receptivity(endometrial thickness,pulsatil⁃ity index(PI),resistance index(RI)),hormone levels(progesterone(P),estradiol(E2),human chorionic gonadotropin(HCG)),immune function(CD3+,CD4+,CD4+/CD8+).Results The scores of TCM symptoms of patients with recurrent spontaneous abortion after treatment were lower than those before treatment(P<0.05);After treatment,the TCM symptom score of the observation group was lower than that of the control group(P<0.05).The clinical effective rate of observation group was higher than that of the control group(95.56%vs 79.55%,P<0.05).The endometrial thickness in patients with recurrent spontaneous abortion was significantly higher after treatment compared to before treatment(P<0.05),and the pulsatility index(PI)and resistance index(RI)were signifi⁃cantly lower after treatment compared to before treatment(P<0.05).The endometrial thickness in the observation group was signifi⁃cantly higher than in the control group after treatment(P<0.05),PI and RI were significantly lower in the observation group than in the control group(P<0.05).The levels of P,E2,and HCG after treatment were higher than those before treatment(P<0.05).After treatment,the levels of P,E2,and HCG in observation group were higher than those in control group(P<0.05).After treatment the levels of CD3+,CD4+,and the CD4
