摘 要:目的观察微生态制剂联合整蛋白型肠内营养剂治疗重症肺炎的效果及对免疫功能与炎性反应的影响。方法选取2021年11月—2024年5月聊城市人民医院老年病科收治的重症肺炎患者80例,根据随机数字表法分为观察组(n=40)和对照组(n=40)。观察组采用微生态制剂联合整蛋白型肠内营养剂治疗,对照组采用整蛋白型肠内营养剂治疗,2组均治疗10~14 d。比较2组临床疗效,治疗前后免疫功能指标、血清炎性因子[白介素-6(IL-6)、C反应蛋白(CRP)]、营养指标[血红蛋白(Hb)、白蛋白(Alb)、前白蛋白(PA)]。结果观察组总有效率为97.50%,高于对照组的82.50%(χ^(2)=5.000,P=0.025)。治疗10~14 d后,2组CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)均较治疗前升高,CD8^(+)均较治疗前降低,且观察组高/低于对照组(P<0.01);2组血清IL-6及CRP水平均较治疗前降低,且观察组低于对照组(P<0.01);2组Hb、Alb及PA水平均较治疗前升高,且观察组高于对照组(P<0.05或P<0.01)。结论微生态制剂联合整蛋白型肠内营养剂治疗重症肺炎能提升效果,改善患者的免疫功能,降低炎性反应,对重症肺炎患者的康复有积极意义。Objective To observe the effect of microecological preparation combined with whole protein enteral nutrition on severe pneumonia and the effect on immune function and inflammatory response.Methods Eighty patients with severe pneumonia admitted to the Department of Geriatrics,Liaocheng People′s Hospital from November 2021 to May 2024 were selected and divided into observation group(n=40)and control group(n=40)according to random number table method.The observation group was treated with microecological preparation combined with whole protein enteral nutrition,the control group was treated with whole protein enteral nutrition.Both groups were treated for 10 to 14 days.The clinical efficacy,immune function indexes,serum inflammatory factors(IL-6,CRP)and nutritional indexes(Hb,Alb,PA)before and after treatment were compared between the two groups.Results The total effective rate of the observation group was 97.50%,which was higher than that of the control group(χ^(2)=5.000,P=0.025).After 10 to 14 days of treatment,CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in two groups were higher than before treatment,CD8^(+)were lower than before treatment,and the observation group was higher/lower than control group(P<0.01).Serum IL-6 and CRP levels in two groups were lower than before treatment,and observation group was lower than control group(P<0.01).The levels of Hb,Alb and PA in two groups were higher than before treatment,and the observation group was higher than the control group(P<0.05 or P<0.01).Conclusion Microecological preparation combined with whole protein enteral nutrition in the treatment of severe pneumonia can improve the effect,improve the immune function of patients,reduce inflammatory response,and have positive significance for the rehabilitation of patients with severe pneumonia.
