天回医简穨、疝、狐病辨证治疗特点探析

摘　　要：以老官山天回医简所载“穨”“疝”“狐”三病为研究对象,运用文献学、训诂学等方法,对其病名内涵及理法方药进行考辨。“穨”专指阴部肿大下坠的病证;“疝”专指腹部疾病,其义有二:一是腹部疼痛伴病变部位凸起如山,二是病由气聚攻冲所致,医简列举肠疝、心疝、女疝3种类型,方剂5首,病机除寒热错杂、痰瘀闭阻,或痰热瘀血互结成实甚或瘀毒互结外,尚有脏腑气血阴阳俱虚,兼感风寒,及阳虚湿阻等;“狐”以病态命名,强调体内突出之物时有时无,时上时下,如狐之出没无定的特点,分狐之阳瘅、狐之阳胀、狐之阴胀、狐之积阴、气狐、笄狐、直狐、阴狐、体狐等9种类型,命名方法多样,如按病机、按症候特点、按发病时间、按发病部位命名等,虽未涉及治法方药,但病机分析准确独到,达到了较高水平。This study examined three medical conditions,“?”(Tui),“疝”(Shan),and“狐”(Hu)recorded in the Bamboo Slip Medical Manuscripts Excavated from Tianhui Township.Using philological and exegetical methods,this study explored the meanings of the disease names and their corresponding diagnostic principles,treatment methods,formulas,and medications.Tui refers to a condition characterized by the swelling and sagging of the genitals.Shan refers to abdominal diseases and has two distinct meanings:(1)abdominal pain with protruding lesions like mountains and(2)disorders caused by accumulation and counterflow of Qi.In the manuscript,Shan has been categorized into three types,namely,intestinal hernia,cardiac hernia,and female-specific hernia,and five prescriptions have been listed.The pathogenesis includes a mix of cold and heat,obstruction by phlegm and blood stasis,the combination of phlegm-heat and blood stasis,or even the combination of stasis and toxins.It also involves deficiencies in zangfu organs,Qi,blood,Yin,and Yang,compounded by wind-cold invasion or Yang deficiency with dampness obstruction.Hu is named based on its pathological condition,emphasizing an intermittent protrusion within the body that shifts in position unpredictably,similar to a fox′s elusive nature.It is divided into nine subtypes,阳瘅(Yang Dan),阳胀(Yang Zhang),阴胀(Yin Zhang),积阴(Ji Yin),气狐(Qi Hu),笄狐(Ji Hu),直狐(Zhi Hu),阴狐(Yin Hu),and体狐(Ti Hu).The naming methods vary,mainly based on pathogenesis,symptom characteristics,time of onset,or affected body location.Although no treatment methods or prescriptions have been provided,its analysis of the pathogenesis of Hu is highly accurate and insightful.
