基于“木郁达之”探析高血压合并心房颤动辨证治疗思路

摘　　要：高血压是心房颤动(Atrial fibrillation,AF)的独立危险因素,有效延缓高血压合并AF的发生及进展,减少卒中、心肌梗死等心血管危险事件的发生是防治的重点任务。基于“木郁达之”理论,从中医学及现代医学角度阐明“木郁”在高血压合并AF的重要地位,灵活运用“木郁”证的治疗法则“达之”,强调“因病致郁-因郁致病”互为因果、恶性循环的重要意义,提出调神达郁、通络畅郁等调摄方法,使高血压合并AF患者得到有效管理。归纳“木郁达之”应用于治疗高血压合并AF的中医防治,为高血压合并AF的中医辨证提供思路。Hypertension is an independent risk factor of Atrial Fibrillation(AF).In the prevention and treatment of hypertension complicated with AF,it is essential to effectively delay the occurrence and development of the complication and to reduce the occurrence of cardiovascular risk events including stroke,and myocardial infarction.In this paper,based on the theory of“facilitating the wood depression”,it is clarified that“wood depression”is an important factor in hypertension complicated with AF.The treatment principle of“wood depression”syndrome should be flexibly applied to achieve“facilitating”while emphasizing the importance of the causal and vicious cycle between“depression caused by illness-depression continuing to cause illness”.Methods were proposed for regulation and adjustment,such as regulating the mind and facilitating depression,for effective management of patients with hypertension and atrial fibrillation.Besides,“facilitating the wood depression”was summarized for traditional Chinese medicine(TCM)treatment of hypertension complicated with AF,providing ideas for TCM syndrome differentiation of the complication.
