摘  要:目的观察荆防颗粒对小鼠急性脊髓损伤后肺部炎性反应的影响及作用机制,为临床采用荆防颗粒治疗脊髓损伤后肺损伤提供实验基础。方法将80只小鼠随机分为假手术组、模型组、低浓度药物组(1 g/kg)、中浓度药物组(2 g/kg)、高浓度药物组(4 g/kg),每组16只。模型组及3个药物组均进行脊髓打击,各浓度药物组按照分组给予相应浓度的荆防颗粒溶液进行灌胃;假手术组和模型组给予相同量的0.9%氯化钠注射液灌胃。术后72 h,采用显微镜观察小鼠肺部的胸膜下出血程度;使用苏木素—伊红染色及TUNEL染色观察各组小鼠肺部形态学改变及细胞凋亡情况;使用肺组织湿干重比值评估小鼠肺部水肿程度;使用酶联免疫吸附法(ELISA)检测小鼠外周血清及支气管肺泡灌洗液中的炎性因子[白介素(IL)-1β、IL-6、IL-10、肿瘤坏死因子-α(TNF-α)]水平。结果各浓度药物组IL-1β、IL-6、TNF-α水平均较模型组降低,IL-10水平较模型组有所提高(P<0.05);各浓度药物组小鼠胸膜下出血程度更低,肺组织切片中病变程度更轻,且较少出现凋亡细胞;高浓度药物组肺组织湿重约为干重的4.23倍,较模型组降低(P<0.05)。结论荆防颗粒能有效减轻脊髓损伤小鼠肺部炎性反应的严重程度,对小鼠的肺组织有保护作用。Objective To observe the effect and mechanism of Jingfang granules on pulmonary inflammatory reaction after acute spinal cord injury in mice,and to provide experimental basis for clinical use of Jingfang granules in the treatment of lung injury after spinal cord injury.Methods Eighty mice were randomly divided into sham operation group,model group,low concentration drug group(1 g/kg),medium concentration drug group(2 g/kg),high concentration drug group(4 g/kg),with 16 mice in each group.The model group and the three drug groups were performed spinal cord percussion,and the drug groups with different concentrations were given Jingfang granules solution by gavage according to the grouping.The sham operation group and the model group were given the same amount of 0.9%sodium chloride injection by gavage.At 72 hours after operation,the degree of subpleural hemorrhage in the lungs of mice was observed by microscope.Hematoxylin eosin staining and TUNEL staining were used to observe the morphological changes and cell apoptosis in the lungs of mice in each group.The ratio of wet to dry weight of lung tissue was used to evaluate the degree of pulmonary edema in mice.ELISA was used to detect the levels of inflammatory factors(IL-1β,IL-6,IL-10,TNF-α)in peripheral serum and bronchoalveolar lavage fluid of mice.Results The levels of IL-1β,IL-6 and TNF-αin the drug groups with different concentrations were lower than those in the model group,and the levels of IL-10 were higher than that in the model group(P<0.05).The degree of subpleural hemorrhage of mice in each concentration of drug group was lower,the degree of lesion in lung tissue sections was lighter,and fewer apoptotic cells appeared.The wet weight of lung tissue in the high concentration drug group was about 4.23 times of the dry weight,which was lower than that in the model group(P<0.05).Conclusion Jingfang granules can effectively reduce the severity of pulmonary inflammatory reaction in mice with spinal cord injury,and has a protective effect on the lung tissue
