温阳止涕汤联合隔姜灸治疗肺气虚寒型慢性鼻窦炎的临床效果

摘　　要：目的观察温阳止涕汤联合隔姜灸治疗肺气虚寒型慢性鼻窦炎(CRS)的临床效果。方法选取2021年1月—2023年8月山东省第二人民医院收治的肺气虚寒型CRS患者88例为研究对象,按随机数字表法分为观察组和对照组各44例。对照组予以常规西药治疗,观察组在对照组基础上加用温阳止涕汤联合隔姜灸治疗,2组均持续治疗4周。比较2组疗效,治疗前后鼻腔纤毛运动能力[黏液纤毛清除速度(MCT)、黏液纤毛清除率(MTR)、糖精清除时间(SCT)]、嗅觉功能评分、中医证候积分、炎性因子[白介素-17(IL-17)、白介素-33(IL-33)、C反应蛋白(CRP)]水平及不良反应。结果观察组总有效率为95.45%,高于对照组的81.82%(χ^(2)=4.062,P=0.044)。治疗4周后,2组MCT、MTR及气味识别能力、辨别能力、感觉阈值、总分高于治疗前,SCT短于治疗前,且观察组高/短于对照组(P<0.01);2组鼻塞、嗅觉减退、气短乏力、自汗畏风积分及IL-17、IL-33、CRP水平均低于治疗前,且观察组低于对照组(P<0.01)。观察组与对照组不良反应总发生率比较差异无统计学意义(6.82%vs.4.55%,P=1.000)。结论温阳止涕汤联合隔姜灸可提高肺气虚寒型CRS患者疗效,增强鼻腔纤毛运动能力,减轻嗅觉障碍,促进症状消失,改善炎性反应,且不良反应少,安全性较高。Objective To observe the clinical effect of Wenyang Zhiti decoction combined with ginger moxibustion in the treatment of chronic sinusitis(CRS)with lung qi deficiency and cold type.Methods A total of 88 patients with CRS of lung qi deficiency and cold type admitted to Shandong Second Provincial General Hospital from January 2021 to August 2023 were selected as the study objects,and were divided into the observation group and the control group with 44 patients in each group according to random number table method.The control group was treated with conventional Western medicine,and the observation group was treated with Wenyang Zhiti decoction combined with moxibustion on ginger based on the control group.Both groups were treated continuously for 4 weeks.The efficacy,the movement ability of nasal cilia(MCT,MTR,SCT),olfactory function score,TCM syndrome score,the levels of inflammatory factors(IL-17,IL-33,CRP)before and after treatment,and adverse reactions of two groups were compared.Results The total effective rate of the observation group was 95.45%,higher than 81.82%of the control group(χ^(2)=4.062,P=0.044).After 4 weeks of treatment,MCT,MTR,odor recognition ability,discrimination ability,sensory threshold and total score were higher than before treatment,SCT were shorter than before treatment in both groups,and the observation group were higher/shorter than control group(P<0.01).The scores of nasal congestion,anosmia,shortness of breath,spontaneous sweating and wind-fear,the levels IL-17,IL-33 and CRP in two groups were lower than before treatment,and observation group were lower than control group(P<0.01).There was no significant difference in the total incidence of adverse reactions between the observation group and the control group(6.82%vs.4.55%,P=1.000).Conclusion Wenyang Zhiti decoction combined with moxibustion on ginger can improve the curative effect of CRS with lung qi deficiency and cold type patients,enhance the movement ability of nasal cilia,alleviate olfaction disorders,promote the disappearance
