环磷腺苷治疗对慢性心力衰竭患者临床疗效、心功能及左室重构的影响

摘　　要：目的:探讨环磷腺苷治疗对慢性心力衰竭(CHF)患者临床疗效、心功能及左室重构的影响.方法:选取2021年3月至2022年1月本院收治的103例CHF患者作为研究对象.按治疗方式,将其分为观察组(n=49,采取环磷腺苷联合常规抗心衰药物治疗)和对照组(n=54,采取常规抗心衰药物治疗).比较两组患者临床疗效,心功能指标[左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、左心室射血分数(LVEF)],左室重构情况[6分钟步行试验(6MWT)、N端-脑钠肽前体(NT-proBNP)],药物不良反应发生率及CHF复发情况.结果:观察组患者治疗总有效率高于对照组,差异有统计学意义(P<0.05);治疗后,观察组患者LVEDD、LVESD低于对照组,LVEF高于对照组,差异均有统计学意义(P<0.05);治疗后,观察组患者6MWT距离长于对照组,NT-proBNP水平低于对照组,差异均有统计学意义(P<0.05);观察组患者药物不良反应总发生率、复发率均低于对照组,差异均有统计学意义(P<0.05).结论:环磷腺苷治疗CHF可取得较好的临床疗效,能有效改善患者心功能与左室重构,且安全性良好.Objective:To explore the effects of cyclic adenosine monophosphate therapy on clinical efficacy,cardiac function and left ventricular remodeling in patients with chronic heart failure(CHF).Methods:A total of 103 CHF patients admitted to our hospital from March 2021 to January 2022 were selected as research subjects.According to the treatment methods,the patients were divided into observation group(n=49,received cyclic adenosine monophosphate with conventional anti-heart failure drugs)and control group(n=54,received conventional anti-heart failure drugs).The clinical efficacy,cardiac function indexes[left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),left ventricular ejection fraction(LVEF)],left ventricular remodeling[6-minute walk test(6MWT),N-terminal pro-brain natriuretic peptide(NT-proBNP)],incidence of adverse drug reactions and CHF recurrence were compared between the two groups.Results:After treatment,the total effective rate of treatment in observation group was higher than that in control group,and the difference was statistically significant(P<0.05).LVEDD and LVESD in observation group were lower than those in control group,and LVEF was higher than that in control group,and the differences were statistically significant(P<0.05).The 6MWT distance in observation group was longer than that in control group,and the level of NT-proBNP was lower than that in control group,and the differences were statistically significant(P<0.05).The total incidence of adverse drug reactions and recurrence rate in observation group were lower than those in control group,and the differences were statistically significant(P<0.05).Conclusion:Cyclic adenosine monophosphate can achieve good clinical efficacy in the treatment of chronic heart failure,and can effectively improve the level of cardiac function and left ventricular remodeling in patients,and the safety is good.
