麦角新碱联合缩宫素对宫缩乏力性产后出血的防治效果

摘　　要：目的:探讨麦角新碱联合缩宫素防治宫缩乏力性产后出血(PPH)的效果.方法:选取2021年2月至2023年2月于本院经阴道分娩的86例伴有宫缩乏力症状产妇作为研究对象.按照随机数字表法,将其分为观察组(n=43,采用麦角新碱联合缩宫素治疗)和对照组(n=43,采用缩宫素治疗).比较两组产妇PPH情况、凝血功能、血清炎性因子水平及不良反应发生情况.结果:观察组产妇2h及24h出血量均少于对照组,差异均有统计学意义(P<0.05);两组产妇PPH发生率比较,差异无统计学意义(P>0.05).产后24h,观察组产妇凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)水平低于对照组,血小板计数(PLT)水平高于对照组,血清C-反应蛋白(CRP)、肿瘤坏死因子(TNF)-α水平低于对照组,差异均有统计学意义(P<0.05).两组产妇不良反应发生率比较,差异无统计学意义(P>0.05).结论:麦角新碱联合缩宫素应用于宫缩乏力自然分娩产妇,可改善凝血功能,减少出血量,且能减轻炎症反应,安全性好.Objective:To explore the effect of ergometrine combined with oxytocin in the prevention and treatment of postpartum hemorrhage(PPH)caused by uterine atony.Methods:A total of 86 vaginal delivery puerpera with symptoms of uterine atony in our hospital from February 2021 to February 2023 were selected as research subjects.According to the random number table method,they were divided into observation group(n=43,treated with ergometrine combined with oxytocin)and control group(n=43,treated with oxytocin).The PPH conditions,coagulation function,inflammatory indicators level and adverse reactions were compared between two groups.Results:The 2-hour and 24-hour bleeding volume of observation group were lower than those of control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of PPH between two groups(P>0.05).At 24 hours postpartum,the levels of prothrombin time(PT)and activated partial thromboplastin time(APTT)in observation group were lower than those in control group,level of platelet count(PLT)in observation group was higher than that in control group,serum levels of C-reactive protein(CRP),and tumor necrosis factor(TNF)-αin observation group were lower than those of control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between two groups(P>0.05).Conclusion:The combination of ergometrine and oxytocin can improve coagulation function,reduce PPH,and alleviate inflammatory reactions caused by bleeding in natural delivery mothers with uterine atony.The combination therapy is safe.
