血清C肽、胰岛素样生长因子-1、脂蛋白α水平与糖尿病肾病患者的相关性分析及临床价值

摘　　要：目的:探讨血清C肽(C-P)、胰岛素样生长因子(IGF)-1、脂蛋白α(Lpα)水平与糖尿病肾病(DN)患者的相关性及临床价值.方法:选取2022年1月至2023年12月本院收治的115例2型糖尿病(T2DM)患者作为研究对象.依据尿微量白蛋白排泄率(UAER),将其分为T2DM组(n=42,UAER<20μg/min)、早期DN组(n=39,UAER≥20μg/min且<200μg/min)及临床DN组(n=34,UAER≥200μg/min).检测比较三组患者C-P、IGF-1及Lpα水平,分析C-P、IGF-1及Lpα与UAER的相关性.结果:临床DN组患者C-P水平低于早期DN组、T2DM组,IGF-1及Lpα水平高于早期DN组、T2DM组;早期DN组患者C-P水平低于T2DM组,IGF-1及Lpα水平高于T2DM组,差异均有统计学意义(P<0.05).经Pearson相关性检验结果显示,C-P与UAER呈负相关,IGF-1及Lpα与UAER呈正相关(P<0.05).结论:DN患者血清C-P水平降低,IGF-1、Lpα水平升高,血清C-P、IGF-1和Lpα水平变化与DN的发生、发展有关.Objective:To explore the correlation and clinical value of serum C-peptide(C-P),insulin-like growth factor-1(IGF-1)and lipoproteinα(Lpα)levels in patients with diabetic nephropathy(DN).Methods:A totao of 115 type 2 diabetes mellitus(T2DM)patients treated in our hospital from January 2022 to December 2023 were selected as research subjects.According to urinary microalbumin excretion rate(URER),they were divided into T2DM group(n=42,UAER<20μg/min)and early DN group(n=39,UAER≥20μg/min and<200μg/min)and clinical DN group(n=34,UAER≥200μg/min).The levels of C-P,IGF-1 and Lpαin the three groups were detected and compared,and the correlation of C-P,IGF-1 and Lpαwith UAER was analyzed.Results:The C-P level in clinical DN group was lower than that in early DN group and T2DM group,while the IGF-1 and Lpαlevels were higher than those in early DN group and T2DM group,and the C-P level in early DN group was lower than that in T2DM group,and the IGF-1 and Lpαlevels were higher than those in T2DM group,and all the differences were statistically significant(P<0.05).The Pearson correlation test showed that C-P was negatively correlated with UAER,while IGF-1 and Lpαwere positively correlated with UAER(P<0.05).Conclusion:Serum C-P levels decreased and IGF-1 and Lpαlevels increased in patients with DN.The level changes of serum C-P,IGF-1 and Lpαwere related to the occurrence and development of DN.
