摘  要:目的分析预测试教学法(pretest teaching method,PTM)在住院医师规范化培训(包括临床微生物标本采集和送检指南相关知识)中的效果。方法选取2018年8月―2019年8月在深圳市第二人民医院培训的363名住培医师作为研究对象,按照不同的入培年份进行分组,将2018级计为对照组(n=178),将2019级记为试验组(n=185)。对照组实施常规的临床轮转培训方法,试验组在对照组的基础上加用PTM教学法。通过对不同知识点进行考核,分析2组在临床应用型相关知识培训方面的教学效果,并比较2组的执业医师技能考试首次通过率。结果试验组的知识总体考核平均得分为(88.73±11.45)分,高于对照组的分数[(83.48±16.07)分],差异有统计学意义(P<0.05)。试验组对尿液标本、脑脊液标本和伤口部位标本的考核正确率均高于对照组,差异有统计学意义(P<0.05)。在20项测试题中,试验组有19项选择题的正确率高于对照组,其中有8项的正确率比较,差异有统计学意义(P<0.05)。试验组的执业医师技能考试首次通过率为97.8%,高于对照组的94.9%。结论在住培医师的临床培训中应用PTM教学法可有效提高住培医师的培训效果,相比于常规的临床轮转培训方法,PTM教学法更具有优势。Objective To explore the teaching effect of pretest teaching method(PTM)in the standardized training of clinical microbiological specimen collection and submission guidelines for residents.Methods A total of 363 residents admitted to Shenzhen Second People’s Hospital from August 2018 to August 2019 were selected as the research objects.According to the year of training,class 2018 was counted as control group(n=178),and class 2019 was counted as experimental group(n=185).The control group adopted the routine clinical rotation training,and the experimental group received PTM teaching on the basis of the control group.Through the examination of different knowledge points,the teaching effect of 2 groups in clinical application related knowledge training was analyzed,and the passing rate of the first time in the medical practitioner skill examination of the two groups was compared.Results The average scores of the experimental group was(88.73±11.45)points,which was higher than that of the control group[(83.48±16.07)points],and the difference was statistically significant(P<0.05).The accuracy of urine,cerebrospinal fluid and wound specimens in experimental group was higher than those of the control group,and the differences were statistically significant(P<0.05).Among the 20 test questions,the correct rate of 19 multiple choice questions in the experimental group was higher than that in the control group,and the correct rate of 8 items was compared,the differences were statistically significant(P<0.05).The first pass rate of the medical licensing examination in the experimental group was 97.8%,which was higher than that in the control group 94.9%.Conclusion The application of PTM teaching method in the clinical training of residential physicians can effectively improve the training effect of residential physicians.Compared with the the routine clinical rotation training,PTM teaching method has more advantages.
