摘 要:目的 探讨孕妇血清同型半胱氨酸(homocysteine, HCY)水平以及红细胞叶酸水平对不良妊娠结局的预测价值。方法 回顾性选取2023年1—12月徐州市妇幼保健院接受孕检并生产的94例孕妇的临床资料,根据其最终的妊娠结局分为不良妊娠组(n=30)、正常组(n=64),检测所有孕妇血液样本中的HCY以及红细胞叶酸水平,构建Logistic回归方程分析HCY、红细胞叶酸水平对妊娠结局的影响,并构建列线图及校准曲线评估预测价值。结果 不良妊娠组血清HCY水平高于正常组,但红细胞叶酸水平低于正常组,差异有统计学意义(P均<0.05)。经绘制ROC曲线,发现血清HCY水平、红细胞叶酸水平的AUC分别是0.757与0.868,其在预测孕妇不良妊娠结局中存在一定价值。结论 血清HCY水平以及红细胞叶酸水平是影响孕妇出现不良妊娠结局的独立因素,且具有一定的预测价值,建议医护人员提高对上述指标的关注,并及时给予药物治疗,降低孕妇出现不良妊娠结局的风险。Objective To explore the predictive value of serum homocysteine(HCY)level and erythrocyte folate level in pregnant women for adverse pregnancy outcome.Methods The clinical data of ninety-four pregnant women who re‐ceived pregnancy examination and gave birth in Xuzhou Maternal and Child Health Hospital from January to Decem‐ber 2023 were retrospectively selected.According to the final pregnancy outcome,they were divided into adverse preg‐nancy group(n=30)and normal group(n=64).The levels of HCY and red blood cell folate in blood samples of all preg‐nant women were detected.Logistic regression equation was constructed to analyze the effects of HCY and red blood cell folate levels on pregnancy outcomes,and a nomogram and calibration curve were constructed to evaluate the pre‐dictive value.Results The level of serum HCY in the adverse pregnancy group was higher than that in the normal group,but the level of red blood cell folate was lower than that in the normal group,and the differences were statisti‐cally significant(all P<0.05).The ROC curve was drawn,and it was found that the AUC of serum HCY level and red blood cell folate level were 0.757 and 0.868,which had certain value in predicting adverse pregnancy outcomes of pregnant women.Conclusion Serum HCY level and red blood cell folate level are independent factors affecting the ad‐verse pregnancy outcomes of pregnant women,and they have certain predictive value.It is suggested that medical staff should pay more attention to the above indicators,and give drug treatment in time to reduce the risk of adverse preg‐nancy outcomes of pregnant women.
