头颈部CTA联合多模态磁共振扫描在急性缺血性脑卒中患者中的诊断价值

摘　　要：目的 探讨头颈部CT血管成像(computed tomography angiography, CTA)联合多模态磁共振扫描(multimodal magnetic resonance imaging, mp-MRI)在急性缺血性脑卒中(acute ischemic stroke, AIS)患者中的诊断价值。方法 目的选择2023年1月—2024年6月东莞市凤岗医院接诊的60例疑似AIS患者为研究对象,均给予头颈部CTA和mp-MRI检查,比较CTA、mp-MRI及两者联合的诊断效能。结果 临床综合诊断结果显示,60例疑似AIS患者中,阳性41例,阴性19例。头颈部CTA检查结果显示,阳性39例,阴性21例。mp-MRI检查结果显示,阳性38例,阴性22例。头颈部CTA联合mp-MRI的灵敏度为95.12%(39/41)、特异度为89.47%(17/19)、准确度为93.33%(56/60),高于头颈部CTA的85.37%(35/41)、78.95%(15/19)、83.33%(50/60)及mpMRI的80.49%(33/41)、73.68%(14/19)、78.33%(47/60),三种方式比较,差异有统计学意义(χ^(2)=4.023、3.577、5.490,P均<0.05)。结论 头颈部CTA联合mp-MRI的诊断价值高于单一诊断,可为临床诊断提供一定的参考依据。Objective To investigate the diagnostic value of head and neck computed tomography angiography(CTA)combined with multimodal magnetic resonance imaging(mp-MRI)in patients with acute ischemic stroke(AIS).Meth⁃ods To objectively select sixty patients with suspected AIS who were admitted to Dongguan Fenggang Hospital from January 2023 to June 2024 as the study subjects.All patients were examined by head and neck CTA and mp-MRI.The diagnostic efficacy of CTA,mp-MRI and their combination was compared.Results The results of clinical compre⁃hensive diagnosis showed that among the sixty suspected AIS patients,forty-one were positive and nineteen were nega⁃tive.The results of CTA examination of head and neck showed that thirty-nine cases were positive and twenty-one cases were negative.The results of mp-MRI showed that thirty-eight cases were positive and twenty-two cases were negative.The sensitivity,specificity and accuracy of head and neck CTA combined with mp-MRI were 95.12%(39/41),89.47%(17/19)and 93.33%(56/60),respectively,which were higher than those 85.37%(35/41),78.95%(15/19),83.33%(50/60)of head and neck CTA and(80.49%(33/41),73.68%(14/19),78.33%(47/60))of mp-MRI,com⁃pared with three methods,the differences were statistically significant(χ^(2)=4.023,3.577,5.490,all P<0.05).Conclu⁃sion The diagnostic value of head and neck CTA combined with mp-MRI is higher than that of single diagnosis,which can provide some reference for clinical diagnosis.
