摘 要:目的 探究CT和磁共振成像(magnetic resonance imaging, MRI)在足踝部病变诊断中的应用价值。方法目的选取2021年3月—2024年3月广东省第二中医院收治的因足踝部疼痛就诊的104例疑似足踝部病变的患者(109患足)为研究对象。所有患者均给予CT和MRI检查,以手术或临床综合诊断结果为“金标准”,比较CT和MRI在足踝部病变中的诊断价值。结果 经手术或临床综合诊断结果显示,104例疑似足踝部病变患者中,阳性36例,阴性68例。CT结果显示,阳性38例,阴性66例。MRI诊断结果显示,阳性42例,阴性62例。CT的灵敏度为77.78%(28/36)、特异度为85.29%(58/68)、阳性预测值为73.68%(28/38)、阴性预测值为87.88%(58/66)。MRI的各项诊断效能值分别为94.44%(34/36)、88.24%(60/68)、80.95%(34/42)、96.77%(60/62)。两种方式的诊断效能值比较,差异无统计学意义(χ^(2)=2.903、3.879、0.584、3.011,P均>0.05)。CT和MRI对软组织病变、关节病变、骨病变的诊断符合率比较,差异无统计学意义(P均>0.05)。结论 CT和MRI在足踝部病变诊断中的总体诊断价值相当。Objective To explore the application value of CT and Magnetic Resonance Imaging(MRI)in the diagnosis of foot and ankle lesions.Methods A total of 104 patients(109 affected feet)with suspected foot and ankle lesions who were admitted to Guangdong Second Traditional Chinese Medicine Hospital from March 2021 to March 2024 due to foot and ankle pain were purposefully selected as study subjects.All patients were examined by CT and MRI.The diagnostic value of CT and MRI in foot and ankle lesions was compared with the"gold standard"of surgical or clinical comprehensive diagnosis.Results The results of surgery or clinical comprehensive diagnosis showed that among the 104 patients with suspected foot and ankle lesions,36 were positive and 68 were negative.CT results showed that 38 cases were positive and 66 cases were negative.The results of MRI diagnosis showed that 42 cases were positive and 62 cases were negative.The sensitivity of CT was 77.78%(28/36),the specificity was 85.29%(58/68),the positive pre‐dictive value was 73.68%(28/38),and the negative predictive value was 87.88%(58/66).The diagnostic efficacy val‐ues of MRI were 94.44%(34/36),88.24%(60/68),80.95%(34/42)and 96.77%(60/62),there were no significant dif‐ferences in the diagnostic efficacy between the two methods(χ^(2)=2.903,3.879,0.584,3.011,all P>0.05).There was no significant difference in the diagnostic coincidence rate of soft tissue lesions,joint lesions and bone lesions between CT and MRI(all P>0.05).Conclusion The overall diagnostic value of CT and MRI in the diagnosis of foot and ankle lesions is comparable.
