糖尿病肾病患者血清SOD、TXB2、内皮素变化及其与肾功能的关系

摘  要:目的探讨糖尿病肾病(diabetic kidney disease,DKD)患者血清超氧化物歧化酶(superoxide dismutase,SOD)、血栓素B2(thromboxaneB2,TXB2)、内皮素-1(endothelin-1,ET-1)、肾功能指标水平与DKD分期的相关性。方法选取2023年5月-2024年6月于贵黔国际医院肾内科就诊的180例DKD患者,根据慢性肾脏病分期标准将患者分为Ⅰ~Ⅱ期组(80例)、Ⅲ~Ⅳ期组(58例)、V期组(42例)。比较3组患者血清相关指标和肾功能指标水平,采用Pearson相关系数分析患者血清相关指标、肾功能指标水平与DKD分期的相关性。结果V期组患者血清SOD水平均低于Ⅰ~Ⅱ期组和Ⅲ~Ⅳ期组,血清TXB2、ET-1、血清肌酐(serum creatinine,Scr)水平均高于Ⅰ~Ⅱ期组和Ⅲ~Ⅳ期组,差异均有统计学意义(P均<0.05)。V期组患者的24 h尿蛋白、尿微量白蛋白/尿肌酐比值(urinary albumin/creatinine ratio,UACR)、尿微量白蛋白排泄率(urinary albumin excretion rate,UAER)均高于Ⅰ~Ⅱ期组和Ⅲ~Ⅳ期组,估算肾小球滤过率(estimated glomerular filtration rate,eGFR)低于Ⅰ~Ⅱ期组和Ⅲ~Ⅳ期组,差异均有统计学意义(P均<0.05)。SOD、eGFR水平与DKD分期呈负相关,TXB2、ET-1、Scr、24 h尿蛋白、UACR、UAER与DKD分期呈正相关。结论DKD患者血清SOD下降,TXB2、ET-1升高,且其水平变化与肾功能严重程度有关。Objective To investigate the correlation between serum superoxide dismutase(SOD),thromboxane B2(TXB2),endothelin-1(ET-1),renal function index levels and diabetic kidney disease(DKD)staging in patients with DKD.Methods A total of 180 patients with DKD who were treated in the Department of Nephrology of Guiqian Inter-national Hospital from May 2023 to June 2024 were selected.According to the staging criteria of chronic kidney dis-ease,the patients were divided into stageⅠ-Ⅱgroup(80 cases),stageⅢ-Ⅳgroup(58 cases)and stage V group(42 cases).The levels of serum related indexes and renal function indexes were compared among the three groups.Pearson correlation coefficient was used to analyze the correlation between serum related indexes,renal function indexes and DKD stage.Results The level of serum SOD in the V stage group was lower than that in theⅠ-Ⅱstage group and theⅢ-Ⅳstage group,and the levels of serum TXB2,ET-1 and serum creatinine(Scr)were higher than those in theⅠ-Ⅱstage group and theⅢ-Ⅳstage group,the differences were statistically significant(all P<0.05).The 24-hour urinary protein,urinary albumin/creatinine ratio(UACR)amd urinary albumin excretion rate(UAER)in the V stage group were higher than those in theⅠ-Ⅱstage group and theⅢ-Ⅳstage group,and the estimated glomerular filtra-tion rate(eGFR)was lower than that in theⅠ-Ⅱstage group and theⅢ-Ⅳstage group,the differences were statisti-cally significant(all P<0.05).The levels of SOD and eGFR were negatively correlated with DKD stage,and TXB2,ET-1,Scr,24-hour urinary protein,UACR and UAERwere positively correlated with DKD stage.Conclusion Serum SOD decreased,TXB2 and ET-1 increased in DKD patients,and their levels were related to the severity of renal function.
