妊娠期孕妇患糖尿病的危险因素分析

摘　　要：目的探究影响妊娠期孕妇患糖尿病的危险因素。方法回顾性选取2022年10月-2023年10月福建省光泽县妇幼保健院收治的60例孕妇的临床资料,根据是否罹患妊娠期糖尿病将孕妇分为正常组(20例)与妊娠期糖尿病组(40例)。比较两组一般资料情况与早孕期代谢指标情况,并采用多因素Logistic回归分析妊娠期孕妇患糖尿病的危险因素。结果两组孕妇糖尿病家族史、早孕期收缩压、早孕期舒张压、空腹血糖、餐后1 h血糖、餐后2 h血糖、早孕期甘油三酯、早孕期低密度脂蛋白胆固醇、早孕期总胆固醇、早孕期高密度脂蛋白胆固醇水平比较,差异均有统计学意义(P均<0.05)。两组孕妇妊娠年龄、妊娠次数、不良孕产史比较,差异均无统计学意义(P均>0.05)。多因素Logistic回归分析显示,有糖尿病家族史、空腹血糖>5.1 mmol/L、餐后1 h血糖>7.8 mmol/L、餐后2 h血糖>6.7 mmol/L、早孕期收缩压>120 mmHg、早孕期舒张压>70 mmHg、早孕期甘油三脂>1 mmol/L、早孕期低密度脂蛋白胆固醇>2.5 mmol/L、早孕期高密度脂蛋白胆固醇<1.5 mmol/L、早孕期总胆固醇>4.2 mmol/L均为孕妇发生妊娠期糖尿病的危险因素(P均<0.05)。结论通过早孕期观察孕妇糖尿病家族史、空腹血糖、餐后1 h血糖、餐后2 h血糖、血压水平与血脂代谢水平,可预测妊娠期糖尿病的发生情况,从而有利于后续针对性产检,减少不良妊娠结局的发生。Objective To explore the risk factors of diabetes mellitus in pregnant women during pregnancy.Methods The clinical data of 60 pregnant women admitted to the Guangze County Maternal and Child Health Hospital in Fujian Province from October 2022 to October 2023 were retrospectively selected.According to whether they had gestational diabetes mellitus,the pregnant women were divided into normal group(20 cases)and gestational diabetes mellitus group(40 cases).The general data and metabolic indexes in early pregnancy were compared between the two groups,and the risk factors of diabetes mellitus in pregnant women were analyzed by multivariate Logistic regression.Results There were significant differences in family history of diabetes,systolic blood pressure in early pregnancy,diastolic blood pressure in early pregnancy,fasting plasma glucose,1-hour postprandial plasma glucose,2-hour postprandial plasma glucose,triglyceride in early pregnancy,low density lipoprotein cholesterol in early pregnancy,total choles-terol in early pregnancy and high density lipoprotein cholesterol in early pregnancy between the two groups(all P<0.05).There were no significant differences in gestational age,number of pregnancies,and history of adverse preg-nancy between the two groups(all P>0.05).Multivariate Logistic regression analysis showed that family history of dia-betes mellitus,fasting plasma glucose>5.1 mmol/L,1-hour postprandial plasma glucose>7.8 mmol/L,2-hour post-prandial plasma glucose>6.7 mmol/L,systolic blood pressure>120 mmHg in early pregnancy,diastolic blood pressure>70 mmHg in early pregnancy,triglyceride>1 mmol/L in early pregnancy,low density lipoprotein cholesterol>2.5 mmol/L in early pregnancy,high density lipoprotein cholesterol<1.5 mmol/L,and total cholesterol>4.2 mmol/L in early preg-nancy in early pregnancy were all risk factors for gestational diabetes mellitus in pregnant women(all P<0.05).Conclusion By observing the family history of diabetes mellitus,fasting blood glucose,1-hour postprandial blood glu-cose,2-hour
