糖尿病肾病临床检验中尿蛋白及尿微量白蛋白的价值研究

摘　　要：目的分析尿蛋白及尿微量白蛋白检测在糖尿病肾病诊断中的临床应用效果。方法选取2023年1月-2024年2月在尤溪县总医院治疗的150例糖尿病肾病患者为研究组,同时选择同期150例单纯糖尿病患者作为对照组,分别进行尿蛋白及尿微量白蛋白的单项检测及联合检测,以肾活检病理诊断为金标准,对比不同检测方式的诊断效能、诊断结果。结果尿蛋白检测检出阳性147例,阴性153例;尿微量白蛋白检测检出阳性为147例,阴性为153例;联合检测检出阳性为151例,阴性149例。联合检测的灵敏度、特异度、准确度、阳性预测值、阴性预测值均高于单项尿蛋白及尿微量白蛋白检测,差异均有统计学意义(P均<0.05)。结论尿蛋白及尿微量白蛋白联合检测能够显著提高糖尿病肾病的诊断准确度。Objective To analyze the clinical application effect of urinary protein and urinary microalbumin detection in the diagnosis of diabetic nephropathy.Methods A total of 150 patients with diabetic nephropathy treated in Youxi County General Hospital from January 2023 to February 2024 were selected as the study group.At the same time,150 patients with simple diabetes were selected as the control group.The single detection and combined detection of uri-nary protein and urinary microalbumin were carried out respectively.The diagnostic efficacy and diagnostic result of different detection methods were compared with the pathological diagnosis of renal biopsy as the gold standard.Results A total of 147 cases were positive and 153 cases were detected in urinary protein test.Urinary microalbumin test was positive in 147 cases and negative in 153 cases.Combined detection confirmed 151 positive cases and 149 negative cases.The sensitivity,specificity,accuacy,positive predictive value and negative predictive value of com-bined detection were higher than those of single urinary protein and urinary microalbumin detection,and the differ-ences were statistically significant(all P<0.05).Conclusion The combined detection of urinary protein and urinary mi-croalbumin can significantly improve the diagnostic accuracy of diabetic nephropathy.
