摘　　要：目的分析血清β2-微球蛋白(β2-microglobulin,β2-MG)、胱抑素C(cystatin C,CysC)结合尿微量白蛋白/肌酐(albumin-to-creatinine ratio,ACR)检测评估2型糖尿病(type 2 diabetes mellitus,T2DM)患者早期肾损伤的效果。方法选取2023年1月-2024年8月盐城市大丰人民医院收治的T2DM患者72例,其中36例单纯T2DM患者为对照组,36例T2DM并早期肾损伤患者为观察组,均测定β2-MG、Cys-C与ACR,比较两组指标水平,并绘制受试者操作特征(receiver operating characteristic,ROC)曲线分析单一与联合指标测定的评估价值。结果观察组β2-MG、Cys-C、ACR水平高于对照组,差异均有统计学意义(P均<0.05)。ROC曲线结果显示:β2-MG、Cys-C、ACR及联合检测T2DM早期肾损伤的曲线下面积分别为0.938、0.772、0.968、0.997,灵敏度分别为80.56%、88.89%、94.44%、97.22%,特异度分别为94.44%、55.56%、97.22%、97.22%。结论通过血清β2-MG、Cys-C结合ACR联合检测有效明确T2DM早期肾损害情况,有助于医师早期诊断和病情监测,对患者疾病诊疗有重要意义。Objective To analyze the effect of serum β2-microglobulin(β2-MG),cystatin C(Cys-C)combined with urinary albumin-to-creatinine ratio(ACR)on early renal injury in patients with type 2 diabetes mellitus(T2DM).Methods A total of 72 patients with T2DM admitted to Yancheng Dafeng People's Hospital from January 2023 to Au-gust 2024 were selected,including 36 patients with simple T2DM as the control group and 36 patients with T2DM and early renal injury as the observation group.The levels ofβ2-MG,Cys-C and ACR were measured,and the levels of the two groups were compared.The receiver operating characteristic(ROC)curve was drawn to analyze the evaluation value of single and combined index determination.Results The levels ofβ2-MG,Cys-C and ACR in the observation group were higher than those in the control group,and the differences were statistically significant(all P<0.05).The results of ROC curve showed that the areas under the curve ofβ2-MG,Cys-C,ACR and combined detection of early renal injury in T2DM were 0.938,0.772,0.968 and 0.997,respectively.The sensitivity was 80.56%,88.89%,94.44%and 97.22%,respectively,and the specificity was 94.44%,55.56%,97.22%and 97.22%,respectively.Conclusion The combination of serumβ2-MG,Cys-C and ACR can effectively identify the early renal damage of T2DM,which is helpful for doctors to early diagnosis and disease monitoring,and has important significance for disease diagnosis and treatment of patients.
