摘　　要：目的分析二甲双胍、胰岛素联合治疗妊娠期糖尿病的临床价值。方法选取2023年1月-2024年1月枣庄市妇幼保健院接收的妊娠期糖尿病患者84例,根据不同治疗方案分为观察组和对照组,对照组(n=42)口服二甲双胍和阿卡波糖片治疗,观察组(n=42)使用二甲双胍、阿卡波糖片和胰岛素联合治疗,对比两组血糖指标水平及不良妊娠结局。结果治疗后,两组空腹血糖、糖化血红蛋白、餐后2 h血糖比较,观察组更低,差异均有统计学意义(P均<0.05)。对照组不良妊娠结局(胎儿窘迫、早产、巨大儿、新生儿低血糖、新生儿呼吸窘迫综合征)发生率较观察组高,差异有统计学意义(P<0.05)。结论胰岛素对妊娠期糖尿病治疗效果确切,可与常规降糖药物联合使用,且联合治疗方案更有利于血糖控制,从而获得良好的临床干预效果。Objective To analyze the clinical value of metformin combined with insulin in the treatment of gestational diabetes mellitus.Methods A total of 84 patients with gestational diabetes mellitus received in Zaozhuang Maternal and Child Health Hospital from January 2023 to January 2024 were selected and divided into observation group and control group according to different treatment regimens.The control group(n=42)was treated with metformin and acar-bose tablets orally,and the observation group(n=42)was treated with metformin,acarbose tablets and insulin.The blood glucose levels and adverse pregnancy outcomes were compared between groups.Results After treatment,the fasting plasma glucose,glycated hemoglobin A1c and 2-hour postprandial plasma glucose in the observation group were lower than the control group,the differences were statistically significant(all P<0.05).The incidence of adverse pregnancy outcomes(fetal distress,premature delivery,macrosomia,neonatal hypoglycemia,neonatal respiratory dis-tress syndrome)in the control group was higher than the observation group,the difference was statistically significant(P<0.05).Conclusion Insulin is effective in the treatment of gestational diabetes mellitus.It can be used in combina-tion with conventional hypoglycemic drugs,and the combined treatment scheme is more conducive to blood glucose control,so as to obtain good clinical intervention effect.
