点拨性动态式反馈护理结合负压辅助闭合技术及rh-bFGF在糖尿病足溃疡伤口床护理中的应用效果评估

摘　　要：目的探讨点拨性动态式反馈护理模式联合负压辅助闭合技术及重组人碱性成纤维细胞生长因子(recombinant human basic fibroblast growth factor,rh-bFGF)在糖尿病足溃疡伤口床管理中的应用效果。方法选择2022年2月-2024年7月在贵港市人民医院接受治疗的Wange分级3~4级糖尿病足溃疡患者共120例,依照护理方法差异分为观察组(n=90)与对照组(n=30)。对照组给予负压辅助闭合及rh-bFGF的贯续辅助治疗,观察组在对照组护理基础上额外给予点拨性动态式反馈护理,对比两组伤口愈合评分、伤口愈合时间、皮肤生长时间、换药次数、疼痛评分与并发症发生率。结果相比于对照组,观察组护理4周后伤口愈合评分、疼痛评分、并发症发生率均更低,差异均有统计学意义(P均<0.05)。相较于对照组,观察组的伤口愈合时间、皮肤生长时间、换药次数均更优,差异均有统计学意义(P均<0.05)。结论在糖尿病足溃疡伤口床护理中,采用负压辅助闭合技术结合rh-bFGF并辅以点拨性动态式反馈护理模式,能够缩短伤口愈合时间及皮肤生长时间,减少换药次数,有效缓解患者疼痛,降低并发症发生率,改善患者的总体预后。Objective To explore the application effect of dynamic feedback nursing mode combined with vacuum-assisted closure technology and recombinant human basic fibroblast growth factor(rh-bFGF)in wound bed manage-ment of diabetic foot ulcer.Methods A total of 120 patients with Wanger grade 3-4 diabetic foot ulcer who were treated in Guigang City People's Hospital from February 2022 to July 2024 were selected.According to different nurs-ing methods,the patients were divided into observation group(n=90)and control group(n=30).The control group was given vacuum-assisted closure and rh-bFGF continuous treatment.The observation group was given additional dy-namic feedback nursing on the basis of the control group.The wound healing score,wound healing time,skin growth time,dressing change times,pain score and complication rate were compared between the two groups.Results Com-pared with the control group,the wound healing score,pain score after 4 weeks of nursing and complication rate of the observation group were lower,and the differences were statistically significant(all P<0.05).The wound healing time,skin growth time and dressing change times in the observation group were better than those in the control group,and the differences were statistically significant(all P<0.05).Conclusion In the wound bed nursing of diabetic foot ulcer,the vacuum-assisted closure technique combined with rh-bFGF and supplemented by the dynamic feedback nursing mode can shorten the wound healing time and skin growth time,reduce the number of dressing changes,effectively re-lieve the pain of patients,reduce the incidence of complications,and improve the overall prognosis of patients.
