血清中性粒细胞-淋巴细胞比值、C反应蛋白水平与糖尿病足感染严重程度的相关性分析

摘　　要：目的探讨血清中性粒细胞-淋巴细胞比值(neutrophil-lymphocyte ratio,NLR)、C反应蛋白(C-reactive protein,CRP)水平与糖尿病足感染(diabetic foot infection,DFI)严重程度的关系。方法回顾性选取2021年1月-2024年8月丹江口市第一医院收治的361例糖尿病足患者的临床资料,依据是否感染分为感染组(262例)和非感染组(99例),比较两组血清NLR、CRP水平;依据Wanger分级法将感染组患者分为轻度组(150例)、中度组(78例)、重度组(34例),比较3组血清NLR、CRP水平,分析其与DFI严重程度的相关性,使用受试者操作特征曲线评估血清NLR、CRP对DFI的诊断效能。结果感染组NLR、CRP均比非感染组高,差异均有统计学意义(P均<0.05)。重度组NLR、CRP水平均高于中度组和轻度组,且中度组均高于轻度组,差异均有统计学意义(P均<0.05)。相关性分析显示,NLR、CRP水平均与DFI严重程度呈正相关。NLR联合CRP检测诊断DFI的曲线下面积、灵敏度、特异度均高于NLR、CRP单项检测。结论血清NLR和CRP水平与DFI严重程度密切相关,二者联合检测可以提高DFI的诊断准确度。Objective To investigate the relationship between serum neutrophil-lymphocyte ratio(NLR),C-reactive protein(CRP)levels and the severity of diabetic foot infection(DFI).Methods The clinical data of 361 patients with diabetic foot admitted to Danjiangkou First Hospital from January 2021 to August 2024 were retrospectively selected.According to whether they were infected,they were divided into infection group(262 cases)and non-infection group(99 cases).The levels of serum NLR and CRP were compared between the two groups.The patients of infection group were divided into mild group(150 cases),moderate group(78 cases)and severe group(34 cases)according to Wanger classification method.The serum NLR and CRP levels of the three groups were compared,and their correlation with the severity of DFI was analyzed.The receiver operating characteristic curve was used to evaluate the diagnostic effi-cacy of serum NLR and CRP in DFI.Results The NLR and CRP in the infection group were higher than those in the non-infection group,and the differences were statistically significant(both P<0.05).The levels of NLR and CRP in the severe group were higher than those in the moderate group and the mild group,and those of the moderate group were higher than the mild group,the differences were statistically significant(both P<0.05).Correlation analysis showed that NLR and CRP levels were positively correlated with the severity of DFI.The area under the curve,sensi-tivity and specificity of NLR combined with CRP in the diagnosis of DFI were higher than those of NLR and CRP alone.Conclusion Serum NLR and CRP levels are closely related to the severity of DFI,and the combined detection of the two can improve the diagnostic accuracy of DFI.
