彩色多普勒超声在老年2型糖尿病下肢血管病变检测中的应用与准确性评估

摘　　要：目的探讨彩色多普勒超声(color Doppler ultrasound,CDU)在老年2型糖尿病(type 2 diabetes mellitus,T2DM)下肢血管病变检测中的应用价值及其准确性。方法选取2023年1月-2024年9月在淄博市市级机关医院就诊的156例老年T2DM疑似并发下肢血管病变患者作为研究对象。所有患者均接受CDU检查,并以临床综合诊断为金标准,对CDU的诊断效能及一致性进行分析。同时,根据临床综合诊断结果,将阳性人群纳入观察组,阴性人群纳入对照组,对比两组患者的下肢血流动力学参数。结果CDU的阳性检出率为58.33%(91/156),诊断灵敏度、特异度、准确度、阳性及阴性预测值分别为93.55%(87/93)、93.65%(59/63)、93.59%(146/156)、95.60%(87/91)、90.77%(59/65);kappa值为0.868,表明与临床综合诊断的一致性较高。观察组股总动脉、腘动脉、足背动脉的血流峰值流速均低于对照组,股总动脉、腘动脉、足背动脉的搏动指数均大于对照组,差异均有统计学意义(P均<0.05)。结论CDU在老年T2DM下肢血管病变的检测中表现出较高的准确度,其诊断效能与临床综合诊断高度一致。Objective To investigate the application value and accuracy of color Doppler ultrasound(CDU)in the de-tection of lower extremity vascular disease in elderly patients with type 2 diabetes mellitus(T2DM).Methods From January 2023 to September 2024,156 elderly patients with suspected T2DM complicated with lower extremity vascu-lar disease who were treated in Zibo Municipal Government Hospital were selected as the research objects.All pa-tients underwent CDU examination,and the diagnostic efficacy and consistency of CDU were analyzed with clinical comprehensive diagnosis as the gold standard.At the same time,according to the clinical comprehensive diagnosis re-sults,the positive population was included in the observation group,and the negative population was included in the control group.The lower extremity hemodynamic parameters of the two groups were compared.Results The positive detection rate of CDU was 58.33%(91/156),and the diagnostic sensitivity,specificity,accuracy,positive and negative predictive values were 93.55%(87/93),93.65%(59/63),93.59%(146/156),95.60%(87/91)and 90.77%(59/65),re-spectively.The kappa value was 0.868,indicating a high consistency with the clinical comprehensive diagnosis.The peak blood flow velocity of common femoral artery,popliteal artery and dorsalis pedis artery in the observation group were lower than those in the control group,and the pulsatility index of common femoral artery,popliteal artery and dor-salis pedis artery were higher than those in the control group,the differences were statistically significant(all P<0.05).Conclusion CDU showed high accuracy in the detection of lower extremity vascular disease in elderly patients with T2DM,and its diagnostic efficacy was highly accuracy with clinical comprehensive diagnosis.
