达格列净与厄贝沙坦联合治疗对早期糖尿病肾病患者尿功能指标及肾功能的影响

摘　　要：目的探讨达格列净与厄贝沙坦联合治疗对早期糖尿病肾病(diabetic nephropathy,DN)患者的影响。方法选择2023年4月-2024年4月河池市第一人民医院收治的120例早期DN患者,按照不同治疗方法分为两组,各60例。对照组予厄贝沙坦单药治疗,观察组在对照组基础上给予达格列净治疗。比较两组临床疗效、肾功能、血糖指标、尿功能、不良反应发生情况。结果观察组治疗总有效率较对照组高,差异有统计学意义(P<0.05)。治疗后,观察组尿转铁蛋白、尿微量白蛋白、尿素氮、血肌酐、空腹血糖、糖化血红蛋白水平、日内平均血糖波动幅度均低于对照组,肾小球滤过率较对照组高,差异均有统计学意义(P均<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论达格列净与厄贝沙坦联合治疗早期DN疗效显著,能够改善患者尿功能及肾功能,并降低血糖水平,且安全性较高。Objective To investigate the effect of dapagliflozin combined with irbesartan on patients with early dia-betic nephropathy(DN).Methods A total of 120 patients with early DN admitted to Hechi First People's Hospital from April 2023 to April 2024 were selected and divided into two groups according to different treatment methods,60 cases in each group.The control group was treated with irbesartan monotherapy,and the observation group was treated with dapagliflozin on the basis of the control group.The clinical efficacy,renal function,blood glucose indexes,uri-nary function and adverse reactions were compared between the two groups.Results The total effective rate of treat-ment in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of transferrin,microalbumin,blood urea nitrogen,serum creatinine,fasting plasma glucose,glycated hemoglobin A1c and mean amplitude of glycemic excursions in the observation group were lower than those in the control group,and the estimated glomerular filtration rate was higher than that in the control group,with statistically significant differences(all P<0.05).There was no significant difference in the incidence of ad-verse reactions between the two groups(P>0.05).Conclusion The combination of dapagliflozin and irbesartan is effective in the treatment of early DN,which can improve the urinary function and renal function of patients,reduce blood glucose levels,and has high safety.
