宫腔镜子宫肌瘤电切术与腹腔镜子宫肌瘤切除术治疗糖尿病伴子宫肌瘤的临床价值对比研究

摘　　要：目的 探讨宫腔镜子宫肌瘤电切术与腹腔镜子宫肌瘤切除术治疗糖尿病伴子宫肌瘤的临床价值。方法纳入2023年6月—2024年11月中国人民解放军陆军第七十三集团军医院收治的100例糖尿病伴子宫肌瘤患者,根据治疗方法不同分为对照组(50例)、研究组(50例)。对照组进行腹腔镜子宫肌瘤切除术,研究组进行宫腔镜子宫肌瘤电切术,对比两组手术指标、性激素指标[黄体生成素(luteinizing hormone,LH)、促卵泡成熟激素(follicle stimulating hormone,FSH)、雌二醇(estradiol,E_(2))、孕酮]、并发症发生率、糖脂代谢水平。结果研究组术中出血量、手术时间均小于对照组,差异均有统计学意义(P均<0.05)。术后,研究组LH、FSH、E2、糖化血红蛋白、空腹血糖、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇水平均优于对照组,差异均有统计学意义(P均<0.05)。研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论 宫腔镜子宫肌瘤电切术治疗糖尿病伴子宫肌瘤,术中出血量、手术时间较优,对患者性激素指标影响较小,有助于保持糖脂代谢水平稳定,减少并发症的发生风险。Objective To explore the clinical value of hysteroscopic myomectomy and laparoscopic myomectomy in the treatment of diabetes mellitus with uterine fibroids.Methods A total of 100 patients with diabetes mellitus and uterine fibroids admitted to the 73rd Army Group Hospital of the Chinese People's Liberation Army from June 2023 to November 2024 were included.According to different treatment methods,they were divided into control group(50cases) and study group(50 cases).The control group underwent laparoscopic myomectomy,and the study group underwent hysteroscopic myomectomy.The surgical indicators,sex hormone indicators [luteinizing hormone(LH),follicle stimulating hormone(FSH),estradiol(E_2),progesterone],complication rate,glucose and lipid metabolism levels were compared between the two groups.Results The intraoperative blood loss and operation time of the study group were less than those of the control group,and the differences were statistically significant(both P<0.05).After operation,the levels of LH,FSH,E_2,glycated hemoglobin A1c,fasting plasma glucose,high density lipoprotein cholesterol and low density lipoprotein cholesterol in the study group were better than those in the control group,and the differences were statistically significant(all P<0.05).The incidence of complications in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Hysteroscopic myomectomy for the treatment of diabetes mellitus with uterine fibroids has better intraoperative blood loss and operation time,and has less impact on the patient's sex hormone index,which helps to maintain a stable level of glucose and lipid metabolism and reduce the risk of complications.
