MCHC联合RDW检验对早期糖尿病肾病的临床意义与诊断价值

摘　　要：目的 探讨红细胞血红蛋白浓度(mean corpuscular hemoglobin concentration,MCHC)联合红细胞分布宽度(red cell distribution width,RDW)检验对早期糖尿病肾病(diabetic nephropathy,DN)的临床意义与诊断价值。方法 选取2022年10月—2023年10月于政和县医院体检确诊的DN早期患者63例为DN组,选取同期体检确诊的单纯糖尿病(diabetes mellitus,DM)患者63例为DM组。采用单因素分析与多因素Logistic回归分析早期DN发生的相关影响因素。绘制受试者操作特征(receiver operating characteristic,ROC)曲线,以评估MCHC、RDW对早期DN的预测价值。结果 两组性别、年龄、文化程度、体重指数、病程、吸烟史、白细胞计数、血小板比较,差异均无统计学意义(P均>0.05)。与DM组比较,DN组MCHC水平更低,RDW水平更高,差异均有统计学意义(P均<0.05)。多因素Logistic回归分析结果显示,MCHC、RDW是早期DN发生的影响因素(P均<0.05)。ROC曲线结果显示,MCHC单项检测、RDW单项检测、联合检测预测早期DN发生的曲线下面积分别为0.740、0.804、0.937。结论 MCHC联合RDW检验对早期DN的临床预测价值较高。Objective To investigate the clinical significance and diagnostic value of mean corpuscular hemoglobin concentration(MCHC) combined with red cell distribution width(RDW) test in early diabetic nephropathy(DN).Methods From October 2022 to October 2023,63 patients with early DN diagnosed by physical examination in Zhenghe County Hospital were selected as DN group,and 63 patients with simple diabetes mellitus(DM) diagnosed by physical examination in the same period were selected as DM group.Univariate analysis and multivariate Logistic regression were used to analyze the related influencing factors of early DN.The receiver operating characteristic(ROC) curve was drawn to evaluate the predictive value of MCHC and RDW for early DN.Results There were no significant differences in gender,age,education level,body mass index,course of disease,smoking history,white blood cell and platelet between the two groups(all P>0.05).Compared with DM group,the level of MCHC was lower and the level of RDW was higher in DN group(both P<0.05).Multivariate Logistic regression analysis showed that MCHC and RDW were the influencing factors of early DN(both P<0.05).ROC curve results showed that the area under the curve of MCHC single detection,RDW single detection and combined detection in predicting early DN was 0.740,0.804and 0.937,respectively.Conclusion MCHC combined with RDW test has a high clinical predictive value for early DN.
