摘 要:目的 研究2型糖尿病老年男性患者糖化血红蛋白(glycated hemoglobin A1c,HbA1c)水平与骨质疏松的相关性。方法 选取2022年5月—2023年5月在淄博市张店区妇幼保健院确诊的2型糖尿病老年男性患者200例为研究对象。根据HbA1c水平分为A组(HbA1c<7%)65例、B组(7%≤HbA1c≤9%)63例、C组(HbA1c>9%)72例。比较3组骨代谢指标、骨密度。Spearman相关性分析HbA1c水平与骨质疏松严重程度的相关性。结果 3组间前胶原I型氨基端肽(procollagen type I N-terminal propeptide,PINP)、胶原降解产物C端肽(Cterminal telopeptide of type I collagen,CTX)比较,C组均大于B组,B组均大于A组;3组间T值比较,A组大于B组,B组大于C组,差异均有统计学意义(P均<0.05)。HbA1c水平与PINP、CTX呈正相关,与T值呈负相关(P均<0.001)。结论 2型糖尿病老年男性患者HbA1c水平与骨质疏松严重程度呈正相关。Objective To study the correlation between glycated hemoglobin A1c(HbA1c) level and osteoporosis in elderly male patients with type 2 diabetes mellitus.Methods A total of 200 elderly male patients with type 2 diabetes mellitus in Zhangdian Maternal and Child Health Hospital from May 2022 to May 2023 were selected as the research objects.According to the HbA1c level,65 cases were divided into group A(HbA1c<7%),group B(7%≤HbA1c≤9%)63 cases,and group C(HbA1c>9%) 72 cases.Bone metabolism indexes and bone mineral density were compared among the three groups.Spearman correlation analysis was used to analyze the correlation between HbA1 c level and the severity of osteoporosis.Results Procollagen type I N-terminal propeptide(PINP) and C-terminal telopeptide of type I collagen(CTX) were compared in the three groups,group C were greater than group B,and group B were greater than group A;the T value of group A was higher than that of group B,and that of group B was higher than that of group C,the differences were statistically significant(all P<0.05).HbA1c level was positively correlated with PINP and CTX,and negatively correlated with T value(all P<0.001).Conclusion The level of HbA1c in elderly male patients with type 2 diabetes mellitus was positively correlated with the severity of osteoporosis.
