摘 要:目的 分析血糖、糖化血红蛋白、尿液微量白蛋白/尿肌酐(albumin/creatinine ratio,ACR)检测在糖尿病肾病诊疗中的作用分析。方法 回顾性选取2023年1月—2024年5月厦门大学附属中山医院收治的560例2型糖尿病肾病患者的临床资料作为研究组,并选取同期560例健康体检者的临床资料作为对照组,两组均进行血糖联合ACR检测、糖化血红蛋白联合ACR检测和血糖、糖化血红蛋、ACR三者联合检查,以肾小球滤过率检测为金标准。对比两组空腹血糖、餐后2 h血糖、糖化血红蛋白、ACR水平、诊断结果和诊断效能。结果对照组空腹血糖、餐后2 h血糖、糖化血红蛋白、ACR水平均低于研究组,差异均有统计学意义(P均<0.05)。血糖联合ACR检出真阳性483例,糖化血红蛋白联合ACR检出真阳性469例,联合检测检出真阳性551例。三者联合检测诊断灵敏度、特异度、准确度均高于单一检测,差异均有统计学意义(P均<0.05)。结论 血糖、糖化血红蛋白联合ACR检测在2型糖尿病肾病诊疗中具有较高的诊断价值。Objective The role of blood glucose,glycated hemoglobin A1c,and albumin/creatinine ratio(ACR) in the diagnosis and treatment of diabetic nephropathy was analyzed.Methods The clinical data of 560 patients with type 2diabetic nephropathy admitted to Zhongshan Hospital Affiliated with Xiamen University from January 2023 to May 2024 were retrospectively selected as the study group,and the clinical data of 560 healthy subjects during the same period were selected as the control group.Both groups were tested for blood glucose combined with ACR,glycated hemoglobin A1c combined with ACR,and blood glucose,glycated hemoglobin A1c,and ACR.The glomerular filtration rate test was the gold standard.The fasting plasma glucose,2-hour postprandial plasma glucose,glycated hemoglobin A1c,ACR level,diagnostic results and diagnostic efficacy were compared between the two groups.Results The levels of fasting plasma glucose,2-hour postprandial plasma glucose,glycated hemoglobin A1c and ACR in the control group were lower than those in the study group,and the differences were statistically significant(all P<0.05).483cases of true positive were detected by blood glucose combined with ACR,469 cases of true positive were detected by glycated hemoglobin A1c combined with ACR,and 551 cases of true positive were detected by combined detection.The diagnostic sensitivity,specificity and accuracy of the combined detection of the three were higher than those of the single detection,and the differences were statistically significant(all P<0.05).Conclusion The detection of blood glucose,glycated hemoglobin A1c combined with ACR has high diagnostic value in the diagnosis and treatment of type 2 diabetic nephropathy.
