探索血清C肽联合糖化血红蛋白诊断2型糖尿病的临床价值

摘　　要：目的 探究2型糖尿病(type 2 diabetes mellitus,T2DM)诊断中采用血清C肽+糖化血红蛋白(glycated hemoglobin A1c,HbA1c)水平检测的价值。方法 选择2023年1月—2024年9月靖江市第二人民医院收治的196例确诊为T2DM的患者(研究组)及同期健康体检者200例(对照组)。所有研究对象均接受血液检测,根据检测结果对比两组血清C肽、HbA1c水平。绘制受试者操作特征(receiver operating characteristic,ROC)曲线分析上述指标联合诊断T2DM的价值。结果 研究组血清C肽水平低于对照组,HbA1c水平高于对照组,差异均有统计学意义(P均<0.05)。血清C肽联合HbA1c诊断T2DM的ROC曲线下面积、灵敏度与特异度均高于各指标单一检测。结论 血清C肽联合HbA1c对T2DM的临床诊断有重要作用,联合诊断的价值较高,可作为临床诊断的依据。Objective To explore the value of serum C-peptide + glycated hemoglobin A1c(HbA1c) level detection in the diagnosis of type 2 diabetes mellitus(T2DM).Methods From January 2023 to September 2024,a total of 196 patients diagnosed with T2DM(study group) and 200 healthy subjects(control group) admitted to Jingjiang Second People's Hospital were selected.All subjects received blood tests,and serum C-peptide and HbA1c levels were compared between the two groups according to the test results.The receiver operating characteristic(ROC) curve was drawn to analyze the value of the above indicators in the combined diagnosis of T2DM.Results The level of serum Cpeptide in the study group was lower than that in the control group,and the level of HbA1c was higher than that in the control group,the differences were statistically significant(both P<0.05).The area under the curve of ROC,sensitivity and specificity of serum C-peptide combined with HbA1c in the diagnosis of T2DM were higher than those of single detection of each index.Conclusion Serum C-peptide combined with HbA1c plays an important role in the clinical diagnosis of T2DM.The value of combined diagnosis is high and can be used as the basis for clinical diagnosis.
